Abstract
----- Using the Ministry of Health, Labour and Welfare's drug price standard item list (as of 13 August 2026, 12,428 items) and Pharmacy Table 4 of the Statistics of Medical Care Activities in Public Health Insurance (claims reviewed in August 2025) — drug points by price class — this paper looked at how skewed drug prices are, and how the skew by number of items differs from the skew by money spent. No product names, manufacturer names, or ingredient names are given. The median price of the 7,064 oral items is 20.1 yen, and items at 500 yen or more are only 0.0810 of them, yet they take 0.3064 of pharmacy oral-drug points. Items under 50 yen are 0.7380 of items and 0.3103 of points, nearly equal to those at 500 yen or more. The 90th ÷ 10th percentile is 42.8 for oral, 172.6 for external, and 836.1 for injections, and for injections, 500 yen or more is 0.6821 of items and 0.9975 of points. What separates them is whether one counts by number of items or by money spent. This paper is a structural interpretation of the shape of numbers in public aggregate data; it is not medical advice, not a diagnosis, and not a treatment recommendation. The author is not a physician. Decisions about individual health should be discussed with a physician or a medical institution. This is a preprint that has not been peer reviewed. No new mathematical theorem and no new law are claimed. It does not judge whether drug prices are high or low, good or bad, how prices are set or the system, or drug efficacy or safety. Product names, manufacturer names, and ingredient names are only counted and never shown. Prices are per standard unit, and the list and the points are one year apart. Prefectures and pharmacies are not compared. Source: processed from the drug price standard item list (Ministry of Health, Labour and Welfare website) and the Statistics of Medical Care Activities in Public Health Insurance (e-Stat). On the making of this work: The ideas and content of this work stem from the author's own considerations. Assistance from an AI (a large language model) was used for structuring, English translation, and checking the algebra. Any remaining errors or misinterpretations are solely the author's. Feedback and corrections are sincerely appreciated.
The results are packaged in the Greenfield Startup Model (GSM), which explains the priority of startups to release the product as quickly as possible, and the need to shorten time-to-market, by speeding up the development through low-precision engineering activities.
