It is demonstrated that glycolytic reprogramming via SO synergizes potently with TGF-β3 to mitigate IVDD progression, and that the S-T@GM microsphere platform represents a promising integrative strategy that simultaneously targets metabolic dysfunction and impaired regeneration for effective intervertebral disc repair.

Abstract

Intervertebral disc degeneration (IVDD) is a leading contributor to low back pain and is characterized by a proinflammatory microenvironment and disruption of extracellular matrix (ECM) metabolic homeostasis. Although aberrant glycolytic reprogramming has been implicated in IVDD pathogenesis, effective therapeutic strategies targeting this metabolic dysregulation remain limited. To address this gap, we developed the injectable gelatin methacrylate (GelMA) microspheres functionalized with sodium oxamate (SO) and co-loaded with transforming growth factor-β3 (TGF-β3) (S-T@GM) to enable the synergistic metabolic and regenerative therapy. Following confirmation of glycolytic abnormalities in human degenerated disc tissues and TNFα-stimulated nucleus pulposus cells (NPCs), we systematically characterized the biocompatibility of S-T@GM microspheres and evaluated their therapeutic efficacy both in vitro and in a rat puncture-induced IVDD model. SO effectively normalized glycolytic flux and attenuated inflammatory responses in NPCs; notably, the combined S-T@GM treatment-unlike either component alone-most efficiently restored ECM homeostasis by enhancing type Ⅱ collagen (COL Ⅱ) synthesis while suppressing the catabolic enzyme ADAMTS5. In vivo, S-T@GM treatment provided superior preservation of disc height, nucleus pulposus hydration, and overall tissue architecture, with immunohistochemical analyses confirming concurrent inflammation suppression and sustained COL Ⅱ retention. Collectively, these findings demonstrate that glycolytic reprogramming via SO synergizes potently with TGF-β3 to mitigate IVDD progression, and that the S-T@GM microsphere platform represents a promising integrative strategy that simultaneously targets metabolic dysfunction and impaired regeneration for effective intervertebral disc repair.

Versatile Microgel Platform for Intervertebral Disc Degeneration Therapy: Targeting Pericyte‐Mediated Fibrosis and Protecting Nucleus Pulposus Cells

This study identified pericytes as novel fibrotic effector cells in IVDD, contributing to fibrosis through activation of the TGF‐β signaling pathway, and developed a microgel platform designed to specifically inhibit pericyte activation and pro‐fibrotic transition.

Ultrasound-triggered piezocatalytic nanotherapy for intervertebral disc regeneration: remodeling the inflammatory milieu and restoring matrix homeostasis.

Intervertebral disc degeneration (IVDD) is the principal cause of chronic low back pain, characterized by extracellular matrix (ECM) catabolism and an exacerbated inflammatory milieu. Given the inherent difficulties in achieving biological restoration through conventional clinical modalities, we herein designed an ultr...

ROS-responsive biomaterial delivery of the TCM-derived small molecule coptisine protects stem cells and attenuates intervertebral disc degeneration

Oxidative stress compromise nucleus pulposus-derived mesenchymal stem cells (NPMSCs) and restrict endogenous repair in intervertebral disc degeneration (IDD). Here, human nucleus pulposus (NP) tissues showed that secreted phosphoprotein 1 (SPP1) expression increased with degeneration severity. Screening of 2,987 tradit...

Injectable ECM-mimetic dynamic Schiff-base hydrogels deliver 5-aminosalicylic acid for immunomodulatory and alleviation of ferroptosis-associated injury of intervertebral disc degeneration.

Intervertebral disc degeneration (IDD) is driven by persistent inflammation, macrophage dysregulation, and ferroptosis-associated injury of nucleus pulposus cells (NPCs). Here, we developed an acid-responsive injectable hydrogel based on oxidized chondroitin sulfate and adipic acid dihydrazide-modified hyaluronic acid...

Intradiscal Delivery of Notoginsenoside R1‐Loaded PLGA Nanoparticles Attenuates Intervertebral Disc Degeneration in Rats

Intervertebral disc degeneration (IVDD) contributes to persistent low back pain, but existing treatments focus on symptom relief rather than addressing the degeneration. Notoginsenoside R1 (NR1), a bioactive saponin derived from Panax notoginseng, has exhibited protective effects in experimental models of disc injury....

Incorporation of Bone Marrow Stem Cell-Derived Exosomes into GelMA Hydrogel Effectively Attenuates Disc Degeneration in the Vertebrae of Rats

Chronic back pain and related disabilities are largely caused by intervertebral disc degeneration (IVDD). However, effective treatments remain limited. Existing treatment modalities, encompassing both conservative and surgical approaches, often fail to effectively arrest disease progression and may be associated with s...
