Abstract
The digital transformation of the Peruvian healthcare system faces challenges relating to interoperability, infrastructure, connectivity and training, which limit the secure and timely exchange of clinical information. The coexistence of incompatible platforms between the Ministry of Health (MINSA), EsSalud, the Armed Forces, the National Police and the private sector fragments care, hinders longitudinal patient follow-up and contributes to the duplication of tests, diagnostic delays, clinical errors and administrative overload. These limitations are more pronounced in rural facilities and those with lower technological capacity, thereby exacerbating regional inequalities. Although recent initiatives, such as the Health Sector Digital Agenda and interoperability testing using HL7-FHIR standards, show progress, their effectiveness requires data governance, common standards, information security, sustainable funding and the involvement of healthcare staff. The challenge lies not merely in digitising processes, but in building an interoperable, assessable and equitable ecosystem centred on the needs of patients and professionals across the country.
The findings indicate progress in the adoption of digital technologies, although challenges persist regarding platform compatibility, data standardisation, connectivity, institutional integration, and the secure management of information.
