Abstract

Health service providers (IPS) in Montería, Córdoba, Colombia, face significant challenges related to the fragmentation, lack of interoperability, and insufficient security of their Electronic Health Records (EHRs), hindering continuity of care and compromising patient safety. Despite the enactment of Law 2015 of 2020, which establishes EHR interoperability at the national level, technical and organizational barriers persist. This article presents a blockchain-based system using Hyper-ledger Fabric technology and oracles to improve interoperability, integrity, and traceability of EHR data across multiple IPS in Montería. We propose a decentralized architecture incorporating smart contracts (chaincode), oracle middleware to connect heterogeneous clinical information systems, and an intuitive interface for healthcare stakeholders. By outlining a performance evaluation plan and addressing limitations—such as connectivity restrictions and regulatory compliance with Law 1581 of 2012—this research contributes to improving clinical data sharing and patient-centered care in the Colombian health system.
