Clinical Factors and PET/CT-Detected Metastatic Status in Breast Cancer Patients: A Retrospective Study

Abstract

Introduction: Breast cancer is a major cause of morbidity and mortality in women. Patient prognosis is strongly influenced by the biological characteristics of the primary tumor, particularly its ability to metastasize. This study aims to describe clinical factors associated with metastasis in breast cancer patients based on PET/CT findings.   Methods: This study used a retrospective analytic design with secondary data from the medical records of 37 breast cancer patients who underwent PET/CT examination. Data were analyzed using the Chi-square test in SPSS to assess the association between clinical factors and metastasis, with a significance level of p < 0.05.   Results: The results showed higher metastasis rates in patients who did not undergo surgery (72.2%, p=0.001) and those without prior therapy (45.8%, p=0.032). Metastasis was also more common in postmenopausal patients (55.6%, p=0.141) and within less than one year from diagnosis to PET/CT examination (60%, p=0.278).   Conclusion: Metastasis in breast cancer patients in this study was more frequently found in postmenopausal patients, those without a history of surgery, those without prior therapy, and within an interval of less than one year from diagnosis to PET/CT examination.

Mapping significant clinical factors to relapse-free survival prognosis in breast cancer patients

This study explores the role of clinical factors in influencing relapse-free survival (RFS) in breast cancer patients through comprehensive survival analysis (SA). The data were derived from a large cohort of breast cancer patients, encompassing clinical variables such as age at diagnosis, tumor size, number of positive lymph nodes, tumor grade, histological subtype, and receipt of chemotherapy and radiotherapy. Analytical methods included Kaplan-Meier plots, log-rank tests, Cox proportional hazards modeling, and feature ranking based on univariate and multivariate log-rank and Cox tests. The results show that patients who received chemotherapy demonstrated a shorter median RFS compared to those who did not (102.7 vs. 145.0 months, p = 0.03), likely due to high-risk patients being selected for chemotherapy. Meanwhile, radiotherapy was associated with improved prognosis (hazard ratio (HR) < 1 in Cox model, p < 10−5), in line with meta-analyses showing reduced breast cancer mortality post-radiotherapy. The contribution maps the key clinical determinants of RFS in breast cancer and highlights the practicality of performing SA.

The Outcome of Breast Cancer Patients Treated with Neoadjuvant Chemotherapy: A Single-Institute Experience

Background and objectives: Neoadjuvant chemotherapy has become a commonly used method of treatment for individuals diagnosed with breast cancer. It involves administering it before surgery or radiation therapy, aiming to shrink tumors and possibly enhance surgical results. This study evaluates the impact of Neoadjuvant chemotherapy in improving patient care and making treatment decisions. Methods: This retrospective study included reviewing the medical records of 141 patients with prespecified eligible inclusion criteria who were diagnosed with primary, non-metastatic breast cancer who received Neoadjuvant chemotherapy at the Azadi Hematology Oncology Center in the Duhok/Kurdistan region of Iraq between 2014 and 2023. Clinicopathological data, treatment details, and patient outcomes were collected from the institute's medical records. The data was collected and analyzed using Statistical Package for Social Sciences. Results: Kaplan-Meier survival analysis revealed that patients with invasive ductal carcinoma demonstrated significantly better survival outcomes compared to those with invasive lobular carcinoma, as indicated by a higher censored rate with statistically significant (P = 0.008). Breast-conserving surgery and axillary treatment were associated with better survival (p < 0.0001), and post-neoadjuvant chemotherapy staging influenced outcomes (p < 0.0001). Triple-negative breast cancer showed the highest pathological complete response rate (40%). Conclusions : This study concluded that Neoadjuvant chemotherapy is reasonable treatment for  Epidermal Growth Factor Receptor-2 positive and triple-negative non-metastatic breast cancer. This study found that breast-conserving surgery was associated with better outcomes compared to mastectomy as most patients were included in this study were eligible for this type of surgery based on the already established criteria.

Survival and Predictors in Patients With Breast Cancer Undergoing Surgery for Spinal Metastases: Insights From a Single‐Center Cohort

Background Breast cancer is the most commonly diagnosed malignancy among women globally, with spinal metastases significantly impacting morbidity and mortality. Surgical intervention plays a crucial role in managing symptomatic spinal lesions, yet prognostic factors and outcomes remain areas of active investigation. This study aims to evaluate survival rates and identify predictors in patients with breast cancer undergoing surgery for spinal metastases. Methods A retrospective analysis was conducted on 97 patients who underwent surgery for spinal breast cancer metastases at a single center over a 12‐year period. Data included epidemiological and preoperative profiles, biological markers, and radiological findings. Survival outcomes were assessed using Kaplan–Meier analysis, and prognostic factors were analyzed via Cox regression. Results Postoperative overall survival rates were 72%, 64%, and 34% at 1, 2, and 5 years, respectively. Significant predictors of survival included tobacco use (HR 1.7, p = 0.025), estrogen receptor (HR 0.224, p = 0.03), triple‐negative subtype (HR 4.1, p < 0.01), and lower Karnofsky scores (HR 0.488, p < 0.01). Spinal instability and preoperative neurological status did not significantly influence survival. Complications occurred in 17% of patients, with no 30‐day mortality. Conclusion This study provides one of the largest single‐center experiences of surgery for spinal metastases from breast cancer in the modern treatment era. We found that overall preoperative performance and biological markers, remains a key predictor of survival, while neurological status, disease spread and spinal instability have no impact than in long‐term survival. These results reflect advances in systemic therapy and surgical care, supporting surgery as an important part of multidisciplinary management for selected patients.

Development and evaluation of clinical models based on ultrasound, serum CA153 and molecular subtype in predicting the efficacy of neoadjuvant chemotherapy in breast cancer

Breast cancer is a threat to health, and reliable predictors for neoadjuvant chemotherapy (NAC) efficacy remain limited. The aim of the present study was to develop a clinical model that combines ultrasonography with clinical and pathological characteristics to predict the effectiveness of NAC in patients with breast cancer prior to surgery. The present retrospective study included patients with pathologically confirmed primary breast cancer who underwent NAC followed by surgery based on predefined inclusion and exclusion criteria. Patients were randomly allocated to a test and a validation set in a 7:3 ratio. Clinical data, comprehensive pathological reports and ultrasound imaging features were collected from the Hospital Information System of Hubei Cancer Hospital (Wuhan, China). Univariate and multivariate logistic regression analyses were conducted to identify independent predictive factors, with significance set at a bilateral P<0.05. The receiver operating characteristic curve was used to assess the performance of the model. The present study included 323 female patients with breast cancer. Univariate analysis indicated significant differences in tumor diameter changes, mass margin and posterior echo patterns (classified as shadow, enhancement or no posterior features), change in serum carbohydrate antigen 153 (CA153) levels, clinical N stage (classified as cN0-cN3), hormone receptor expression, Ki-67 levels and molecular type between the pathological complete response (pCR) and pathological incomplete response (non-pCR) group. Multivariate analysis identified margin, molecular subtype, changes in tumor diameter and serum CA153 levels as significant predictors included in the final model (P<0.05). Furthermore, the present study model was validated in the validation set. The area under the curve for the predictive model in test set was 0.823 (95% confidence interval, 0.765–0.881), while in the validation set, it was 0.884 (95% confidence interval, 0.816–0.953). The integrated model, which incorporated ultrasound, serum CA153 levels and molecular subtypes, demonstrated notable potential for preoperative assessment of NAC efficacy in the future.
