Clinicopathological Analysis of Prostate Cancer in Young Men: Experience of a Tertiary Referral Hospital in Western Romania

Abstract

Background and Objectives: Prostate cancer (PC) has traditionally been regarded as a disease of older men, most commonly diagnosed after the age of 70 years. Recent epidemiological data indicate an increasing incidence of this malignancy among men younger than 55 years, with important clinical and therapeutic implications. The present study aimed to analyze the epidemiological, clinical, and pathological characteristics of PC in young patients diagnosed at a tertiary referral urology center in western Romania. Materials and Methods: Medical records and the electronic database of the “Pius Brînzeu” County Emergency Clinical Hospital in Timișoara (PBCECHT) were reviewed to identify men aged ≤55 years diagnosed with PC over a 10-year period (2015–2024). Collected data included: the age at diagnosis, serum prostate-specific antigen (PSA) levels, specimen type, histologic type, Gleason score/International Society of Urological Pathology (ISUP) grade group, tumor stage, and additional prognostic factors. Results: Of the 1742 patients diagnosed with PC during the study period, 70 (4.02%) were aged ≤55 years. The average age at diagnosis for the group of young men was 52.47 years. Most patients were from urban areas (64.29%). The PSA was available in 51/70 cases (range: 1.35–5182; median: 15; IQR: 51). The mean Gleason score was 7.15, and the mean ISUP grade group was 2.5. Histologically, acinar adenocarcinoma predominated, while a minority of cases showed mixed acinar and ductal/adenocarcinomas with ductal features. Most tumors (70%) were clinically significant. Conclusions: Although less common, PC in young men often exhibits aggressive features. It is crucial to consider genetic risk factors and a multidisciplinary approach for optimal management.

Clinicopathological profile of male breast cancer: an institutional experience from Western Bihar, India

Background: Male breast carcinoma (MBC) is rare (~1% of breast diagnoses) and often presents at advanced stages due to limited awareness, screening guidelines, and medical guidance. Data from Bihar, India, remains scarce. This study assessed institutional accrual alongside demographic, clinical, histological, and stage profiles at Netaji Subhas Medical College and Hospital (NSMCH), Bihta, Patna. Methods: An ambispective observational cohort study (January 2019 to December 2025) histopathologically confirmed primary male breast cancer (MBC) cases were included (retrospective: 2019–2023; prospective: 2024–2025). Age, symptoms, histological subtype, and AJCC 8th Edition (pTNM) stage were recorded. Data were analyzed using descriptive statistics with exact binomial and Poisson confidence intervals. Results: Over seven years, 36 patients were identified (mean accrual: 5.14 cases/year), predominantly aged 60–84 years. Painless breast lump was the chief symptom in 28 patients (77.7%). Invasive ductal carcinoma (no special type) was the predominant variant (33 cases, 91.7%). Advanced Stage III or IV disease was present in 27 patients (75.0%) at initial diagnosis. Conclusions: MBC in western Bihar primarily affects older men and usually presented at advanced stages through symptom-driven detection, dominated by invasive ductal carcinoma. Improving public awareness, screening efficiency, rapid histopathological diagnosis, referral networks, and regional cancer registration is essential.

The young breast cancer patient: A tertiary institution’s experience

Background: Breast cancer (BC) in sub-Saharan Africa affects not only older females but also those aged ≤ 40 years. At Chris Hani Baragwanath Academic Hospital (CHBAH), females with suspicious breast lesions undergo combined mammography and ultrasound with integrated Breast Imaging Reporting and Data System (BI-RADS) assessment to guide biopsy. The diagnostic performance of this approach in younger females remains clinically relevant because of increased breast density. Aim: To assess the concordance between bimodal BI-RADS classification and histopathological diagnosis in females aged ≤ 40 years with biopsy-confirmed BC. Setting: Chris Hani Baragwanath Academic Hospital, Johannesburg, South Africa, using data from the South African Breast Cancer and HIV Outcomes (SABCHO) cohort. Methods: A retrospective review of females aged ≤ 40 years diagnosed with invasive BC over seven years was performed. Clinical and demographic characteristics were described, and concordance between bimodal BI-RADS classification and histopathological diagnosis was assessed. Results: Of 354 patients identified, 309 met the inclusion criteria. The mean age was 35.2 ± 3.9 years, and invasive ductal carcinoma was the predominant subtype (93.5%). Concordance between bimodal BI-RADS classification and histopathological diagnosis was 97.7%. BI-RADS 5 was the most common imaging category, while discordant cases were confined to stage I, stage II and stage III disease. Most patients presented with locally advanced disease. Conclusion: Bimodal BI-RADS assessment showed high concordance with histopathological diagnosis in females aged ≤ 40 years with biopsy-confirmed BC, supporting the use of combined mammography and ultrasound for evaluating suspicious breast lesions in this setting. Contribution: This study adds evidence on the concordance of bimodal BI-RADS assessment with histopathological diagnosis in young females with BC in sub-Saharan Africa.

Diagnostic and Prognostic Aspects of Breast Cancer in Young Women at the Thiès Regional Hospital Center, Senegal

Introduction: Breast cancer in young women represents an emerging public health concern. Its impact is often poorly understood and underestimated. Objectives: This study aimed to investigate the diagnostic and prognostic aspects of breast cancer in women aged 40 years or younger who were treated at the Regional Hospital Center of Thiès. Patients and Methods: This retrospective cross-sectional study was conducted from January 5, 2017, to June 8, 2023. It included 101 women aged 40 years or younger with histologically confirmed breast cancer who received care at the Regional Hospital Center of Thiès. Results: The mean age of the patients was 34 years, and the mean delay to consultation was 10.53 months. The disease was predominantly locally advanced: 56.4% of patients had T4 disease, 73.3% had lymph node involvement, and 24.75% had metastases at diagnosis. Invasive ductal carcinoma of no special type was the most frequent histological type, accounting for 96.04% of cases, and SBR grade II was observed in 62.4%. Immunohistochemistry identified luminal B disease in six patients (5.94%) and triple-negative disease in four (3.96%). All patients received chemotherapy; 89.11% received neoadjuvant chemotherapy and 7.9% received adjuvant chemotherapy. Surgery was performed in 44 patients (43.56%). Radiotherapy was administered to 14 patients (13.86%), and hormone therapy was indicated for nine patients (8.9%). Thirty patients had died by the end of the study. Overall survival was 65.6% at 3 years and 60.1% at 5 years. Conclusion: Breast cancer is increasingly prevalent among young women and presents with a more aggressive profile and a poorer prognosis than in older women. Management remains challenging and may be complicated by diagnostic delays, poor therapeutic adherence, and a persistently low survival rate.

Clinical Features and Etiology of Patients Having Advanced Stage Hepatocellular Carcinoma on Initial Presentation

Chronic viral hepatitis, especially HCV infection, was the primary risk factor in this group of patients for advanced-stage HCC, which is an indication of the need for better surveillance, early hepatopathy screening, and timely intervention measures to minimize late-stage presentation and hepatopathy-related mortality.
