Impact of Biopsy-to-Radical Prostatectomy Interval on Adverse Pathological Outcomes in High-Risk Localized and Locally Advanced Prostate Cancer

Abstract

Background: In high-risk prostate cancer, the optimal timing of radical prostatectomy after diagnostic biopsy remains a clinically important issue. A real-world delay between diagnosis and radical treatment frequently occurs, raising concerns about potential disease progression in high-risk patients. Methods: We conducted a single-center retrospective study including patients with high-risk localized and locally advanced prostate cancer who underwent radical prostatectomy from January 2016 to January 2026. Patients were stratified according to biopsy-to-radical prostatectomy interval into two groups: <90 days or ≥90 days. Adverse pathological outcomes were defined as extraprostatic extension, seminal vesicle involvement, positive surgical margins and lymph-node involvement. Univariable comparisons and multivariable logistic regression analyses were performed to identify independent predictors of adverse pathology. Results: A total of 158 patients with high-risk prostate cancer were included, of whom 67 (42.4%) underwent open- or laparoscopic radical prostatectomy within 90 days after biopsy and 91 (57.6%) after ≥90 days. On univariable analysis, the rates of extraprostatic extension were 59.7% vs. 63.7% in the <90-day and ≥90-day groups (p = 0.606), seminal vesicle involvement was observed in 22.4% vs. 24.2% (p = 0.627), positive surgical margins in 35.8% vs. 39.6% (p = 0.632), and lymph node involvement in 6% vs. 5.5% (p = 0.999). In multivariable logistic regression, a biopsy-to-radical prostatectomy interval ≥90 days was not independently associated with extraprostatic extension (OR 1.38, 95% CI 0.68–2.77, p = 0.371), seminal vesicle involvement (OR 1.43, 95% CI 0.63–3.27, p = 0.391) or positive surgical margins (OR 1.31, 95% CI 0.66–2.62, p = 0.443). The number of positive biopsy cores independently predicted extraprostatic extension (OR 1.17, 95% CI 1.04–1.32, p = 0.008), while higher PSA independently predicted seminal vesicle involvement (OR 1.05, 95% CI 1.01–1.10, p = 0.025) and positive surgical margins (OR 1.05, 95% CI 1.00–1.10, p = 0.027). Conclusions: In this real-world cohort of patients with high-risk prostate cancer undergoing radical prostatectomy, no statistically significant independent association was detected between a biopsy-to-radical prostatectomy interval ≥90 days and extraprostatic extension, seminal vesicle involvement, or positive surgical margins. However, the confidence intervals remained compatible with potentially clinically meaningful differences, and residual confounding from clinician-driven prioritisation and unmeasured preoperative factors cannot be excluded. Therefore, these findings should not be interpreted as demonstrating equivalence or the safety of delaying surgery.

Radical Prostatectomy Without Prior Biopsy? Final Histopathology of Radical Prostatectomies in 103 Consecutive Biopsy-naïve Patients Who Underwent Radical Prostatectomy.

BACKGROUND AND OBJECTIVE Prostate biopsy for histological confirmation is the standard before radical prostatectomy (RP). However, some patients refuse biopsy despite highly suspicious imaging findings. This study aimed to evaluate final pathology, assess potential active surveillance (AS) eligibility, and exploratorily evaluate perioperative outcomes and short-term oncological biochemical recurrence (BCR)-free survival after biopsy-naïve RP (bnRP). METHODS We retrospectively analyzed a single-center cohort of patients who underwent bnRP with pT2-pT3 disease between 2019 and 2026. Final pathology was assessed, and potential AS eligibility was approximated using European Association of Urology-based low-risk and favorable intermediate-risk categories. Perioperative outcomes and short-term BCR-free survival were exploratorily compared between patients who underwent bnRP and those who underwent biopsy-confirmed RP (bcRP) using Kaplan-Meier estimation, log-rank testing, and a univariable Cox proportional hazards model. KEY FINDINGS AND LIMITATIONS In 103 consecutive bnRP specimens, prostate cancer was confirmed in all patients. Overall, 94% had clinically significant prostate cancer (csPCa). Compared with 3081 consecutive patients who underwent bcRP, patients who underwent bnRP had fewer low-grade tumors and more locally advanced disease. Overall, 5.8% of the biopsy-naïve patients had International Society of Urological Pathology grade group 1 disease, but only one patient fulfilled the stricter AS approximation criteria. However, in total, 18% of the biopsy-naïve patients fulfilled broader AS criteria and might have warranted formal AS assessment. Exploratory analysis revealed no significant difference in short-term BCR-free survival between patients who underwent bnRP and those who underwent bcRP. CONCLUSIONS AND CLINICAL IMPLICATIONS In our single-center observational analysis of carefully selected patients, bnRP seemed safe and was associated with a high likelihood of csPCa. Nevertheless, the retrospective approximation of AS eligibility highlights that bnRP should be performed cautiously. Exploratory perioperative and short-term oncological analyses need prospective validation.

Association of PI-RADS Score with Clinically Significant Prostate Cancer and Histopathological Upgrading in Patients Undergoing Radical Prostatectomy: A Retrospective Single-Centre Study

Background and Objectives: Multiparametric magnetic resonance imaging (mpMRI) is widely used in prostate cancer diagnosis, but biopsy findings may still underestimate tumor aggressiveness. This study evaluated the association of Prostate Imaging Reporting and Data System (PI-RADS) assessment with clinically significant prostate cancer (csPCa) and histopathological upgrading after radical prostatectomy. Methods: The retrospective analysis comprised 564 patients who underwent mpMRI–transrectal ultrasound fusion-guided biopsy followed by radical prostatectomy between 2021 and 2025. All included patients subsequently underwent radical prostatectomy, so the cohort represents a surgically selected population. Clinical variables, PI-RADS score, biopsy ISUP Grade Group, and final prostatectomy pathology were assessed. Clinically significant disease was defined as ISUP Grade Group ≥ 2. Histopathological upgrading was defined as an increase of at least one ISUP Grade Group between biopsy and final prostatectomy pathology. Results: csPCa was found in 385 patients (68.3%). Patients with csPCa had higher prostate-specific antigen density (PSAD) values than those without csPCa. PI-RADS score was significantly associated with csPCa and remained independently associated with csPCa in multivariable analysis. Histopathological upgrading occurred in 164 patients (29.1%), and PI-RADS score was also independently associated with upgrading. Model discrimination was limited. Conclusions: Preoperative mpMRI with PI-RADS assessment was associated with both csPCa and biopsy underestimation. Although PI-RADS was independently associated with both outcomes, its modest discriminatory performance indicates that it should be interpreted together with other clinical and pathological parameters rather than as a standalone predictive tool.

Biopsy Gleason pattern 4 burden refines preoperative risk stratification in prostate cancer.

PURPOSE To evaluate the prognostic value of biopsy-based Gleason pattern 4 (GP4) burden in a racially and ethnically diverse cohort of men with intermediate- and high-risk prostate cancer undergoing radical prostatectomy. MATERIALS AND METHODS We retrospectively identified 368 men with biopsy-proven Gleason grade group (GG) 2 to 4 prostate cancer treated with radical prostatectomy (RP) between 2017 and 2024. GP4 burden was quantified using multiple biopsy-based measures. Primary outcomes were adverse surgical pathology (ASP) at prostatectomy-extraprostatic extension, seminal vesicle invasion, or lymph node invasion-and biochemical recurrence (BCR) within 3 years. Associations were evaluated using multivariable regression; model discrimination was assessed using Harrell's concordance index. A point-based pattern 4 risk score (P4RS) was developed and compared with an established preoperative clinical risk nomogram for prediction of early biochemical recurrence. RESULTS The cohort was racially and ethnically diverse (76% Black or Hispanic/Latino). ASP occurred in 38% of patients and 17% experienced BCR within 3 years of surgery. ASP risk rose 15% per 2-millimeter increase in total length of pattern 4 on biopsy. Among patients with ≤15 mm GP4, 3-year BCR risk rose 22% per 2-millimeter increase; precision was limited beyond this range. Absolute GP4 metrics improved discrimination for BCR (concordance index 0.716-0.723) relative to GG (0.690). Among risk stratification models, the P4RS demonstrated the highest discrimination (concordance index 0.728). CONCLUSIONS Absolute Gleason pattern 4 burden, particularly total length, provides clinically meaningful prognostic information beyond grade group and improves risk stratification for adverse pathology and early oncologic outcomes after prostatectomy.
