Novel methods to broaden the scope of landmark analysis by directly modeling the restricted residual mean survival time (RRMST) at each landmark date while accommodating dependent censoring are proposed.

Abstract

Dynamic risk assessment is an important tool for healthcare providers to inform treatment selection and optimize patient outcomes. Frequent evaluation of clinical status provides invaluable information on adverse event risk and/or treatment urgency, based on the changes in predictive biomarkers. Landmark analysis is a useful approach for dynamic risk assessment that obviates the complexity of jointly modeling longitudinal covariates and time-to-event outcome. Traditional landmark methods often rely on the proportional hazards assumption and assume independent censoring. We propose novel methods to broaden the scope of landmark analysis by directly modeling the restricted residual mean survival time (RRMST) at each landmark date while accommodating dependent censoring. Advantages of RMST models include removing the assumption that the (hazard) model is correct at every time point; RMST models instead consider a single capped survival time. Moreover, investigators may prefer the area under the survival curve to the hazard function as a metric in terms of interpretability. We derive the asymptotic properties of our proposed estimators and validate their performance in finite samples through extensive simulations. We apply the proposed methods to analyze outcomes under natural end-stage liver disease (ESLD) history (i.e., in the absence of liver transplantation).

A functional landmark flexible-hazards cure model for individual dynamic prediction

Important biomarkers are routinely measured during patients’ follow-up visits to monitor their response to treatment and track disease progression. Accurate, individualized dynamic prediction based on data available at these visits is essential for making informed decisions regarding the next stage of treatment. We pro...

Beyond Fixed Restriction Time: Adaptive Restricted Mean Survival Time Methods in Clinical Trials

This work proposes a novel data-driven, adaptive procedure that identifies the optimal restriction time from a continuous range by maximizing a criterion balancing effect size and estimation precision, and provides a rigorous theoretical foundation that accounts for the additional variability introduced by adaptive sel...

Surrogate-powered Causal Inference with Censored Outcomes

Clinical trials with survival endpoints lose information when participants are censored before death is observed. We develop target-preserving estimators that use posttreatment disease history, such as recurrence or progression, to recover information lost to censoring for marginal survival and restricted mean survival...

Joint Model for Assessing the Effect of Frailty and Longitudinal Measures in Time-To-Event Outcome.

Joint modeling provides a robust statistical framework for simultaneously modeling longitudinal covariate and time-to-event outcome. Apart from the interdependence between longitudinal and survival outcomes there may exist some unobserved random heterogeneity which can be modeled using a joint frailty model (JFM). Unli...

Dynamic prediction intervals for survival times

Most work on survival prediction focuses on estimating survival probabilities rather than predicting individual event times. Recent conformal methods have made it possible to construct prediction intervals for survival times with right-censored outcomes, but existing approaches are restricted to settings with covariate...

Direct Learning of Treatment-Benefit Rankings for Restricted Mean Survival Time in Randomized Trials with Censoring

A direct ranking approach for conditional RMST treatment benefit in randomized trials with right-censored outcomes is proposed that combines orthogonal survival learning with pairwise ranking and may be particularly useful in precision medicine settings where treatment prioritization and patient selection are the prima...
