This is the first reported IVUS-guided UNICORN leaflet modification during TAVR-in-TAVR, highlighting its potential to improve procedural precision and safety in patients at high risk of coronary obstruction.
Abstract
Background Coronary obstruction is a rare but potentially fatal complication of transcatheter aortic valve replacement (TAVR), particularly during valve-in-valve and TAVR-in-TAVR procedures. Leaflet modification techniques have emerged to reduce this risk. Case Summary A 67-year-old woman with severe degeneration of a 26-mm SAPIEN 3 transcatheter heart valve presented with acute heart failure. Computed tomography demonstrated severe prosthetic stenosis and a high risk of left coronary obstruction (virtual valve-to-coronary distance 3.4 mm). Redo TAVR was performed using intravascular ultrasound (IVUS)-guided UNICORN (Undermining Iatrogenic Coronary Obstruction with Radiofrequency Needle) leaflet modification, followed by implantation of a 23-mm RESILIA valve. IVUS enabled precise leaflet traversal and real-time assessment of the relationship between the modified leaflet, valve frame, and left main coronary artery, resulting in an excellent procedural and hemodynamic outcome. Conclusions This is the first reported IVUS-guided UNICORN leaflet modification during TAVR-in-TAVR, highlighting its potential to improve procedural precision and safety in patients at high risk of coronary obstruction.
Redo transcatheter aortic valve implantation (TAVI) is increasingly performed for structural valve deterioration of transcatheter heart valves. Sinus sequestration is a mechanism of coronary obstruction in which displaced leaflets of the index transcatheter valve isolate the coronary sinus from the aortic root af...

Abstract Background Transcatheter aortic valve implantation (TAVI) in bicuspid aortic valve (BAV) poses unique challenges. We report a case in which an apparently benign anatomy resulted in failure of conventional delivery strategies and required a novel balloon-assisted bailout technique. Case summary A 59-year-old ma...

Background Acute leaflet dysfunction after transcatheter pulmonary valve (TPV) implantation is a rare but clinically important complication that remains poorly characterized. Case Summary We report 2 cases of acute stuck leaflet causing significant pulmonary regurgitation immediately postimplantation, the first such ca...

This is among the first reports combining planned mechanical circulatory support with BATMAN-assisted valve-in-valve TMVR with planned IABP support to facilitate safe treatment of selected patients with severe pulmonary hypertension and predicted LVOT obstruction.

Background Concomitant aortic and mitral regurgitation (CAMR) is a rare valvulopathy, seen in patients with scoliosis, and rarely involves aortic regurgitation with concomitant primary mitral regurgitation associated with poor prognosis when left untreated. Current guidelines lack robust evidence to guide the managemen...

Acute coronary obstruction is a rare but potentially fatal complication of transcatheter aortic valve implantation (TAVI), most commonly resulting from displacement of a calcified native leaflet. We describe an unusual mechanism involving delamination of native aortic leaflet tissue with subsequent coronary ostia...
