TL;DR

This is among the first reports combining planned mechanical circulatory support with BATMAN-assisted valve-in-valve TMVR with planned IABP support to facilitate safe treatment of selected patients with severe pulmonary hypertension and predicted LVOT obstruction.

Abstract

Background Left ventricular outflow tract (LVOT) obstruction is a feared complication of transcatheter mitral valve replacement (TMVR), and balloon-assisted translocation of the mitral anterior leaflet (BATMAN) has emerged as a mitigation strategy. Patients with severe pulmonary hypertension are at risk, as abrupt elimination of mitral regurgitation (MR) can destabilize hemodynamics. Case Summary A 78-year-old woman with a degenerated 25-mm surgical bioprosthesis, severe pulmonary hypertension (mean pulmonary artery pressure 65 mm Hg), and reduced cardiac index (1.9 L/min/m2) underwent transseptal BATMAN-assisted valve-in-valve TMVR using a 23-mm SAPIEN 3 Resilia valve with planned intra-aortic balloon pump (IABP) support. A computed tomography–predicted neo-LVOT area of 181 mm2 guided anterior leaflet modification. MR was eliminated with a mean transmitral gradient of 1 mm Hg, no LVOT obstruction, and stable periprocedural hemodynamics. Discussion BATMAN enabled successful TMVR despite high-risk anatomy. Prophylactic IABP facilitated hemodynamic stability during the transition from chronic MR physiology. This is among the first reports combining planned mechanical circulatory support with BATMAN-assisted TMVR in severe pulmonary hypertension. Take-Home Message BATMAN-assisted valve-in-valve TMVR with planned IABP support may facilitate safe treatment of selected patients with severe pulmonary hypertension and predicted LVOT obstruction.

Prevention of Left Ventricular Outflow Tract Obstruction During High-Risk Transcatheter Mitral Valve Replacement.

BACKGROUND Transcatheter mitral valve replacement can carry a left ventricular outflow tract (LVOT) obstruction in patients with small neo-LVOT. CASE SUMMARY A 74-year-old woman with a degenerated 27-mm surgical mitral bioprosthesis presented with severe symptomatic mitral stenosis and biventricular failure. She was...

Severe dynamic left-ventricular outflow-tract obstruction unmasked by transcatheter aortic valve implantation: a case report

Abstract Background Relieving the fixed afterload of severe aortic stenosis can unmask a previously concealed dynamic outflow-tract obstruction. Case summary A 69-year-old man with severe aortic stenosis (valve area 0.65 cm2; 109/64 mmHg), mild concentric hypertrophy (septum 15 mm), and no baseline systolic anterior mo...

Reversing Regurgitant Physiology: Pressure-Volume Insights After Dedicated TAVR for Pure Aortic Regurgitation.

BACKGROUND Dedicated transcatheter aortic valve replacement (TAVR) devices for the treatment of pure aortic regurgitation (AR) have emerged as an alternative to surgery for patients at high or prohibitive operative risk. However, the immediate hemodynamic effects of TAVR in this setting remain poorly characterized. C...

Totally endoscopic septal myectomy combined with mitral valve replacement for suicide left ventricle following transcatheter aortic valve implantation: a case report

Abstract Background Dynamic left ventricular outflow tract obstruction (LVOTO) causing acute haemodynamic collapse is a rare but life-threatening complication after transcatheter aortic valve implantation (TAVI). Known as suicide left ventricle (SLV), it results from abrupt afterload reduction leading to hypercontracti...

Changes in right ventricular-pulmonary artery coupling after transcatheter edge-to-edge repair in primary and atrial mitral regurgitation.

BACKGROUND Right ventricular-pulmonary artery (RV-PA) coupling is an established marker of prognosis in mitral regurgitation (MR), but data on its change after transcatheter edge-to-edge mitral valve repair (M-TEER) in primary or atrial MR are limited. METHODS RV-PA coupling was evaluated in 190 patients undergoing M...
