Abstract

Objective

To investigate positive results on a screening instrument for autism spectrum disorder (pos-ASDS) and associated neonatal and early developmental risk factors in school-age children born extremely preterm (EPT). STUDY

Design

Researchers assessed 380 EPT children (GA 24 to <28 weeks) at 6-7 years-of-age using the Social Communication Questionnaire (SCQ). Pos-ASDS was defined as an SCQ score ≥15. Logistic regression examined independent associations of perinatal factors and 18-22 months behavior and cognitive scores with pos-ASDS.

Results

Fourteen % of the children scored ≥ 15 in the ASDS (SCQ). Risk factors included male sex, lower birth weight, severe retinopathy of prematurity, motor/sensory impairments, lower cognitive scores, poorer emotional competence, and more behavioral problems at 18-22 months. Neonatal MRI and cranial ultrasound findings were not significantly different.

Conclusion

Children born EPT have increased positive ASDS. It is important to assess their socioemotional development early and pos-ASDS risk factors to facilitate prompt intervention.

Early autism risk factors: a cohort study of children born very preterm at 5-years

OBJECTIVE: Explore the association of prenatal, perinatal, and postnatal risk factors with autism risk at 5-years corrected age in very preterm (VPT) children. STUDY DESIGN: Data from a longitudinal cohort study of 315 VPT and 172 term-born children at a US-based academic medical center were analyzed, exploring antecedents of autism risk using elastic net regression. RESULTS: More children born VPT (8.9%) had a Social Communication Questionnaire score associated with autism (i.e., SCQ ≥ 15) than term-born controls (2.9%; p = 0.012). For VPT children, cerebellar abnormalities at term-equivalent age (OR = 1.339, 95% CI = 1.010–1.778) and moderate-severe histologic chorioamnionitis (OR = 4.148, CI = 1.294–13.368) were significantly associated with autism risk, in addition to male sex and social risk. VPT children with SCQ ≥ 15 had poorer adaptive, behavioral, cognitive, and executive functioning (all ps < 0.001). CONCLUSION: Early screening for children with these risk factors can facilitate early intervention for those at most risk for poorer neurodevelopmental outcomes.

[Quality of life of parents of children with autism spectrum disorders].

OBJECTIVE To establish a correlation between the severity of clinical symptoms in children diagnosed with autism spectrum disorders (ASDs) and the quality of life experienced by their parents, and identify potential targets for effective interventions in care. MATERIAL AND METHODS The study included a sample of 40 parents of children diagnosed with ASD and 37 parents of neurotypical children. The clinical and psychometric evaluation of the children with ASD was conducted using the Childhood Autism Rating Scale (CARS). Additionally, the Quality of Life in Autism Questionnaire (QoLA) was used to assess parents' quality of life. Statistical analyses were performed using MS Excel and STATISTICA software, employing nonparametric methods. RESULTS The psychometric assessment using the CARS scale yielded an average score of 22.6±5.8 for the examined children, indicating a mild autism classification. The total QoLA score for parents of children with ASD was 132.6±25.8 points, compared with 171.8±23.6 points for parents of neurotypical children (p<0.001). The quality of life of parents, as assessed by Part B of the QoLA scale, was found to be correlated with the severity of ASD as measured by the CARS scale (p<0.05). The primary symptoms identified that negatively impacted the quality of life of parents included the social impairments associated with autism, challenges in collaborative engagement, a lack of comprehension regarding social rules and roles, alongside communication disorders (p<0.05). CONCLUSIONS The findings highlight potential targets for social and therapeutic interventions with families, which may alleviate parental distress and enhance the quality of life for parents of children diagnosed with ASD.

Development and validation of an early screening tool for young children with autism spectrum disorders: the checklist for Chinese toddlers with autism

Preliminary evidence indicates that the Checklist for Chinese Toddlers with Autism exhibits sound psychometric properties and may serve as an early screening tool for Chinese children with ASD aged 12–24 months, however, external validation is still required to establish its generalizability.

Burden on the caregivers of children and adolescents with autism spectrum disorder: which variables are related?

ABSTRACT Purpose To evaluate the factors associated with the burden of caregivers of children and adolescents with autism spectrum disorder (ASD), identifying which variables have the greatest impact on their daily lives. Methods The study evaluated 19 caregivers of children with ASD, aged 2 to 12 years; 18 were boys (94.7%). The diagnosis was made by a multidisciplinary team, and the participants were under the care of a pediatric neurologist. The variables collected included the child's age and sex, comorbidities (intellectual disability, attention-deficit/hyperactivity disorder, and so forth), current complaints, type of ongoing intervention, score on the Childhood Autism Rating Scale (CARS), caregiver age, number of children, and family income. Caregiver burden was assessed using the Zarit Burden Interview. Statistical analysis used linear regression with a stepwise selection method to identify the factors significantly associated with burden. Results only the CARS score was significantly associated with caregiver burden (p = 0.0077), indicating that higher levels of impairment in ASD are related to greater burden. The patients’ age and sex, comorbidities, current complaints, intervention at the time of assessment, and socioeconomic level were not significant. Conclusion The severity of ASD symptoms, reflected by the CARS score, was the main factor associated with caregiver burden. Other aspects, such as age, comorbidities, and socioeconomic level, may also exert influence, as described in the literature, but were not statistically significantly associated in this study.

The Interplay of Interventions and Social Determinants of Health on Neurodevelopmental Outcomes in Children With Congenital Heart Disease.

BACKGROUND Neurodevelopmental disability is a chief concern of children with congenital heart disease longitudinally. Delays in cognition and language based on neurodevelopmental testing are known morbidities. Although it is known that school-age children often require special education (SPED) services, a closer look at this population correlating interventions and other environmental factors to developmental and school outcomes has not been explored. METHODS Demographic and clinical data were obtained on children who were seen in a cardiac developmental outcomes program, who underwent cardiac surgery in the first year of life, and who had neurodevelopmental testing using the Bayley Scales for Infant and Toddler Development, third edition, at 36 months or prior (n = 216). Receipt of therapies, daycare, educational programming, preschool, and SPED at school age were noted, as well as age at school entry. RESULTS Of 216 children with Bayley Scales for Infant and Toddler Development, third edition scoring, higher maternal education was associated with higher cognitive and language scores. Receipt of early childhood intervention (ECI) was associated with lower cognitive scores. Of the 216 children, 165 had documentation of whether SPED was or was not received. 55.8% of these children received SPED, with high Child Opportunity Index levels serving as a protective factor and ECI speech therapy and preschool SPED (ECSE) increasing the likelihood of receipt of SPED services. CONCLUSION Social determinants of health such as maternal education and neighborhood opportunity are key influencing factors on neurodevelopmental outcomes and receipt of SPED at school age when examining different sociodemographic factors and interventions.
