Core clinical competency indicators for master of nursing specialist (anaesthesia) students in China: a delphi study.

Abstract

Background

Competency training is essential for cultivating qualified anaesthesia Master of Nursing Specialist (MNS) graduates. Although a previous study has established relevant competency dimensions for anaesthesia nursing, no refined framework with operationalized indicators is currently available for Chinese MNS anaesthesia students.

Objective

To construct an index system for the core clinical competencies of MNS anaesthesia students.

Methods

Following literature screening, 26 eligible publications were selected to draft preliminary indicators. Semi-structured interviews with 15 participants were performed, and interview data were analysed via thematic coding.21 experts from 11 regions across China completed two rounds of Delphi surveys. Indicators were rated on a 5-point Likert scale, with expert consensus determined by predefined statistical cut-offs (mean importance score ≥ 3.5, coefficient of variation (CV) ≤ 0.25, ≥ 75% ratings ≥ 4) combined with comprehensive expert revision opinions.

Results

The response rates of the two Delphi rounds were 100% and 95.24%, respectively. The expert authority coefficients (Cr) were 0.95 and 0.96, indicating high reliability of the consultation results. In the second round, Kendall's concordance coefficients (W) for the first, second, and third-level indicators were 0.552 (strong consensus), 0.324 (moderate consensus), and 0.254 (weak-moderate consensus) respectively (P < 0.01), which indicated that the consistency of expert opinions was statistically significant and the consultation results were valid. The range of coefficient of variation was from 0.05 to 0.25. The final indicator system included 8 first-level indicators, 39 s-level indicators, and 109 third-level indicators.

Conclusion

Core clinical competency indicators for MNS anaesthesia students, established in this study, achieved expert consensus validity. These competency indicators can serve as a theoretical reference for the future training and assessment of MNS postgraduates in this field.

Core Competencies and Associated Factors Among Clinical Nurse Educators.

BACKGROUND Clinical nurse educators (CNEs) in China are required to maintain clinical nursing care while undertaking educational management responsibilities, but evidence on their competency profile remains limited. PURPOSE Assess the core competencies of CNEs and identify associated factors. METHODS A cross-sectional survey was conducted with CNEs from hospitals in China using convenience sampling. RESULTS Among the 898 responses, the total competency score was moderate; clinical nursing skills scored highest, and innovation and research competency scored lowest. Competency differed by region, age, professional title, clinical work experience, years as a CNE, initial education, and specialized training (all P < 0.05). Pre-appointment training for clinical teaching was the strongest and most consistent predictor. CONCLUSIONS CNEs reported moderate competency, with the greatest gap in innovation and research. Pre-appointment training was consistently associated with higher competency, highlighting the need for structured preparation for teaching and ongoing professional development.

Development and validation of professional competency scale (PCS) for newly graduated nurses.

AIM To develop the Professional Competency Scale (PCS) with validation evidence to assess professional competency in newly graduated nurses. BACKGROUND Newly graduated nurses' competency affects retention. However, there is a dearth of reliable and valid measures assessing professional competency. DESIGN A methodological study to develop and validate a scale. METHODS The PCS was developed following the nine procedural steps proposed by DeVellis and Thorpe. A total of 650 newly graduated nurses were recruited from hospitals of different levels across Taiwan. Structural validity was tested using exploratory and confirmatory factor analysis and criterion-related validity was also evaluated. Internal consistency reliability was assessed with Cronbach's α and McDonald's ω. RESULTS An initial 51-item questionnaire was developed. Exploratory factor analysis indicated the removal of 11 items, leaving 40 items across three factors of professional competency for newly graduated nurses: clinical care, soft skills and education and management (44% of variance explained). Confirmatory factor analysis results supported the three-factor structure with 40 items. Criterion-related validity was supported by positive relationships with work adaptation (r = 0.59-0.76; p < .001). The PCS demonstrated high internal consistency (α = 0.96; ω = 0.96). CONCLUSION A 40-item PCS for newly graduated nurses in Taiwan was developed and demonstrated good psychometric characteristics.

Construction and practice of a multi-station Chinese medicine specialist nurse training system based on core competencies: a Delphi study

Background With the increasing global recognition of traditional Chinese medicine (TCM), the standardized training of TCM specialist nurses remains challenging due to the lack of competency-based training frameworks that effectively integrate theoretical education with clinical practice. This study aimed to develop and evaluate a competency-based multi-station training system for TCM specialist nurses. Methods A preliminary training framework was developed based on literature review, expert discussions, and the core competency framework of TCM specialist nurses. A three-round Delphi consultation involving 21 experts was conducted to refine the indicators, and the analytic hierarchy process was used to determine indicator weights. The effectiveness of the training system was evaluated among 40 trainees through pre- and post-training assessments of core competencies and satisfaction. Results The Delphi consultation showed high expert authority and consensus, with authority coefficients ranging from 0.90 to 0.92. The final system comprised 6 primary indicators, 23 secondary indicators, and 122 tertiary indicators, with satisfactory consistency of indicator weights (CR < 0.10). After training, trainees demonstrated significant improvements in core competencies across multiple domains, including professional knowledge, clinical practice, management, innovation, teaching, communication, and ethical literacy (P < 0.05). High satisfaction with the training system was also reported. Conclusion The competency-based multi-station training system provides a structured and practical approach for cultivating TCM specialist nurses. By integrating theoretical learning, clinical practice, and competency assessment, this model may contribute to the standardization of TCM nursing education and enhance nursing quality.

Construction of a comprehensive competency evaluation index system for clinical nursing instructors based on a multifaceted feedback model

This study constructed a comprehensive competency evaluation Index system for clinical nursing instructors based on a Multifaceted Feedback Model, which can provide a scientific reference for the recruitment, appointment and competency assessment of clinical nursing teachers. Based on the multifaceted feedback model, the initial draft of the evaluation index system was formulated through literature review, mixed-method research, and group discussions. The expert consultation method was then used to refine and finalize the evaluation system. The weight of each indicator was determined using the Analytic Hierarchy Process (AHP), and a consistency test was conducted to ensure reliability. Two rounds of expert consultations were conducted. The expert response rates were 100% and 93%, the authority coefficients were 0.922 and 0.929, and the Kendall's coefficients of concordance were 0.175 and 0.235, respectively. The final evaluation index system for the competencies of clinical nursing instructors consists of four first-level indicators, twelve second-level indicators, and fifty-six third-level indicators. The comprehensive competency evaluation indicator system for clinical nursing instructors constructed in this study establishes a foundation for the recruitment, training, performance appraisal, competency evaluation and standardized management of clinical nursing teachers. Moreover, this system provides theoretical and practical references for cultivating high-quality nursing talents that meet the development requirements of new medical disciplines and the reform objectives of the medical and health system.

A Competency-Based Evaluation Index System for Recertification of Breast Specialist Nurses: A Delphi Study in China

Introduction Clinical nurse specialists have crucial roles in team development, patient safety, and work quality of nurses. Breast specialist nurses in China are instrumental in managing breast disorders and providing care for patients experiencing complications. However, despite the growing demand for breast specialty care, there are no unified national recertification standards for breast specialist nurses in China, and relevant research in this area remains scarce. This study aimed to develop an evaluation index system for breast specialist nurse recertification and establish a scientific and objective basis for standardizing such systems. Methods Based on the theory of post competency, literature analysis and a two-round Delphi method were used to prepare a recertification evaluation system for breast specialist nurses. Eighteen nursing experts from 14 tertiary hospitals in Guangdong Province participated, selected based on predefined criteria (≥10 years of clinical experience, intermediate/advanced title, and bachelor’s degree or above). Indicator weights at all levels were determined using the proportion allocation method. The mean importance score was ≥ 3.50, and the coefficient of variation (CV) was ≤ 0.25. Results The authority coefficients for these rounds were 0.936 and 0.933, and the Kendall coefficients of concordance were 0.245 and 0.406, respectively (P < 0.001). The final evaluation system comprised 7 first-level, 28 second-level, and 105 third-level indicators, with content features specific to breast specialty care—notably, integrative Chinese and Western nursing, lymphedema management, and multidisciplinary team (MDT) collaboration. Among the seven first-level indicators, clinical practice skills, clinical expertise, and critical thinking skills formed a cluster of core clinical competencies with comparable weights, reflecting the balanced emphasis on technical proficiency, specialist knowledge, and clinical judgment. This Delphi study was conducted and reported in accordance with the Conducting and REporting of DElphi Studies (CREDES) guidelines. Conclusion The recertification evaluation index system developed here is comprehensive, scientifically robust, and practically applicable. It is a basis for nursing managers to assess the competency and recertification of breast specialist nurses in Guangdong Province, China, supporting the quality and safety of breast cancer care. Furthermore, it offers a reference for future recertification efforts beyond the regional context.
