Malignant neoplasms of the reproductive system organs in women in the Kemerovo Region – Kuzbass: incidence, mortality, impact on life expectancy

Abstract

Introduction. The incidence of malignant neoplasms of the reproductive system organs is one of the indicators of women’s reproductive health – an important component of population reproduction.Materials and methods. A comparative analysis of the overall incidence rates of malignant neoplasms of the reproductive system organs and mortality from them in women in the Russian Federation and the Kemerovo Region – Kuzbass, as well as a graphical analysis of age-specific mortality rates from breast cancer, cervical cancer, uterine body cancer, and ovarian cancer were conducted. The contribution of mortality in certain age-related groups from cancer of the reproductive system organs of the leading locations to the differences in women’s life expectancy and the elimination reserves associated with its exclusion for increasing life expectancy was revealed.Results. In the Kemerovo Region – Kuzbass, as in the Russian Federation as a whole, tumors of the female reproductive system are leading in the structure of firstly diagnosed malignant neoplasms and related mortality. The most common oncopathology in women is breast cancer, the incidence of which has increased by 37.5% over the past 10 years in the Kemerovo Region. Reproductive system diseases account for 32% of the causes of death in women from oncopathology of all locations. In Kuzbass, cervical cancer causes the greatest irreversible losses among 30–49 years women, and breast cancer among women aged of 50 years and older. The elimination reserve for increasing life expectancy for women in Kuzbass, associated with the excluding of mortality from breast cancer, is 0.31 years, cervical cancer – 0.20 years, ovarian cancer – 0.11 years, and endometrial cancer – 0.10 years.Limitations. This study is limited by the territories selected for comparison (the Russian Federation and the Kemerovo Region – Kuzbass) and the current lack of availability of demographic data (age-specific mortality rates are available up to 2022, overall mortality rates from malignant neoplasms – up to 2023, overall incidence rates of malignant neoplasms – up to and including 2024).Conclusion. The obtained results are recommended for use in elaborating programs for the prevention and early detection of malignant neoplasms of the female reproductive system organs.Compliance with ethical standards. EPIDEMIOLOGICAL ANALYSIS OF THYROID CANCER AND THE SPECIFICS OF MEDICAL CARE TO THE POPULATION OF THE ARCTIC REGIONS OF NORTHWESTERN RUSSIA (ARCHANGELSK REGION)

Relevance. High morbidity and mortality from oncological pathology is an urgent problem of public health. Current trends in the epidemiological situation around the world do not reflect particularly positive forecasts, even though new methods of early diagnosis and treatment of such patients are being developed. The proportion of thyroid cancer among oncological neoplasms of the endocrine system is high and continues to increase. The purpose of the study is to conduct an epidemiological assessment of thyroid cancer among the population of the Arkhangelsk region in order to determine further prospects for providing medical care to patients. Materials and methods. The analysis was carried out on the basis of information materials from the database of the Unified Interdepartmental Information and Statistical System. The main epidemiological indicators of thyroid cancer have been studied: primary morbidity, mortality, and detection at various stages. A comparison of their changes over the studied time period with the all-Russian values is carried out. Results and their discussion. In general, the epidemiological features of thyroid cancer in the Arkhangelsk region have been determined in comparison with the national values. Thus, the incidence in the region is higher relative to the Russian Federation as a whole. These differences were especially significant among the female population. The trend towards an increase in morbidity has been determined over the past 10 years. At the same time, the regional mortality rate from thyroid cancer is lower compared to the national one. The dynamic mortality curve has been undulating; recent years have been characterized by an increase in. A positive aspect was the relatively high detection rate of thyroid cancer in the initial stages, which is attributed to the developing cancer service system in the region. Conclusions. Despite significant progress in the work of the cancer service in the Arkhangelsk region, there are several problems that require operational decisions.

Analysis of cancer incidence and mortality rates of staff at plutonium production of siberian chemical plant

Purpose: To analyze the oncological morbidity and mortality of the personnel of the plutonium production of the Siberian Chemical Plant. Material and methods: An assessment of the indicators and standardized relative risk of oncological morbidity and mortality of the personnel of the plutonium production of the Siberian Chemical Plant was performed. Cases of malignant neoplasms and deaths due to them in the period 1950–2020 were analyzed for the personnel hired in the period 1950–2015. Results: In the structure of oncological morbidity and mortality among plutonium production personnel in the period 1950–2020, malignant neoplasms of the digestive organs, respiratory system, urinary tract, genital organs, and mammary gland (in women) were the leading ones. Among the female personnel of the study group, the risks of developing malignant neoplasms with the corresponding total dose of external radiation (mSv) were statistically significantly increased for the digestive organs (> 0 mSv, > 20–50 mSv), respiratory system (> 0 mSv, > 0–20 mSv), skin (> 0 mSv), mammary gland (> 0 mSv, > 20–50 mSv), genitourinary organs (> 0 mSv), as well as malignant neoplasms of all localizations combined (> 0 mSv, > 20–50 mSv). In male personnel, the risk of death due to malignant neoplasms was significantly increased in the respiratory system (> 150–200 mSv and > 300–500 mSv), skin (> 20–50 mSv), and urinary tract (> 20–50 mSv). Male personnel in the study group also had a statistically significantly increased risk of death due to malignant neoplasms (with a corresponding total external radiation dose, mSv) of the digestive system (> 100–150 mSv and > 150–200 mSv), respiratory system (> 300–500 mSv), urinary tract (> 0 mSv, > 20–50 mSv), and genitals (> 0 mSv and > 300–500 mSv). Among female personnel – digestive organs (> 0 mSv, > 20–50 mSv), respiration (> 0 mSv, > 0–20 mSv), mammary glands (> 0 mSv, > 20–50 mSv), genital organs (> 0 mSv) and all localizations combined (> 0 mSv, > 20–50 mSv). Conclusion: Male personnel are characterized by an increased risk of developing malignant neoplasms at doses above 150 mSv (for the respiratory system) and mortality due to malignant neoplasms at doses above 300 mSv (for the respiratory system) and at doses above 150 mSv (for the digestive system). For all other identified malignant neoplasm sites in men, as well as in women, the increased risks of morbidity and mortality are predominantly in the range of up to 50 mSv.

The state of cancer care for gastric cancer (C16) in Russia: a population-based analysis of survival and treatment effectiveness considering age and sex characteristics. Part 2 (clinical-population study)

It was found that one-year observed survival of GC patients increased from 37.4% (2000–2004) to 45.8% (2020–2022), an increase of 22.5%, and the necessity of integrating population-based cancer registries into official statistics is substantiated.
