TL;DR

Circulatory proteome is a promising tool to capture distinct tissue-specific responses that correlate with disease activity and tissue injury in patients with IIM.

Abstract

Objective

Tissue-specific biomarkers that are reliable and associated with clinical manifestations of idiopathic inflammatory myopathy (IIM) are lacking. The blood circulation in individuals serves as a central conduit, allowing communication between tissues and facilitating clearance and recycling of tissue-derived proteins. Thus, we applied a dual antibody-based proximity extension assay to profile >1000 proteins in subtypes of patients with IIM.

Methods

Plasma samples from 201 patients diagnosed with IIM at Karolinska University Hospital were analyzed using Olink Proximity Extension Assay technology. Clinical manifestations, disease activity measurements, and laboratory results were collected. First, we evaluated differential protein abundance across IIM subtypes. Then we calculated tissue scores for each disease subtype using tissue gene expression databases. Finally, interferon (IFN) pathway scores were calculated.

Results

Plasma from patients with dermatomyositis was enriched for IFN signalling and skin associated proteins, anti-synthetase syndrome (ASyS) for lung associated proteins, immune-mediated necrotizing myopathy (IMNM) for muscle associated proteins, and inclusion body myositis for T cell response proteins. Patients with IMNM showed the highest tissue score for skeletal muscles, and ASyS for lung tissue score. Skeletal muscle scores correlated strongly with muscle disease activity visual analog scale scores, creatine kinase levels, and muscle weakness score. Patients with interstitial lung disease along with anti-MDA5 and those with anti-Jo1 autoantibodies showed differential enrichment of surfactant proteins and IFN pathway activation.

Conclusions

Circulatory proteome is a promising tool to capture distinct tissue-specific responses that correlate with disease activity and tissue injury in patients with IIM.

FABP3, a Candidate Biomarker of Muscle Strength and Myositis Disease Activity in Patients with Idiopathic Inflammatory Myopathies

Background: Identifying novel, reliable blood-based biomarkers is essential to better assess disease activity in idiopathic inflammatory myopathies (IIMs). We aimed to determine serum fatty acid binding protein 3 (FABP3) levels in IIM patients and evaluated their correlations with disease activity, muscle strength, and...

Serum circulating cell-free messenger RNA profile response to risdiplam treatment in adult patients with late-onset spinal muscular atrophy

Abstract SMN-enhancing therapies have improved outcomes in adult late-onset spinal muscular atrophy (loSMA), but minimally invasive biomarkers that reflect systemic disease and treatment effects are lacking. In this retrospective cohort study, we profiled circulating cell-free messenger RNA (ccfmRNA) in serum samples f...

Markers of microvascular instability predict severity and survival in idiopathic pulmonary fibrosis.

INTRODUCTION Most research on idiopathic pulmonary fibrosis (IPF) has focused on the interplay among fibroblasts, the immune system and epithelial cells. There is growing evidence that microvascular dysfunction also plays a role in disease progression, but large human translational studies are lacking. In this research...

Subphenotype-specific serum proteomic signatures in patients with ARDS

Acute respiratory distress syndrome (ARDS) is associated with a substantial burden of morbidity and mortality. Previous studies have identified reproducible hyperinflammatory and hypoinflammatory ARDS subphenotypes with distinct clinical outcomes and treatment responses. Although recent transcriptomic studies have prov...

S100 proteins in kidney disease: Inflammatory biomarkers, pathogenic mechanisms and therapeutic prospects (Review).

The S100 protein family, particularly the calprotectin heterodimer S100A8/A9, is increasingly recognized as both an inflammatory readout and a context‑dependent mediator of kidney injury. Selected extracellular S100 proteins function as damage‑associated molecular patterns released during cellular stress, leukocyte act...
