CURRENT THERAPEUTIC APPROACHES IN THE TREATMENT OF PERITONEAL CARCINOMATOSIS

TL;DR

In gastric cancer with peritoneal metastasis, normothermic intraperitoneal chemotherapy (NIPS) combined with systemic treatment has demonstrated improved overall survival and higher rates of negative cytology conversion.

Abstract

Peritoneal carcinomatosis (PC) is a neoplastic spread associated with various primary tumors, with a prognosis that varies according to the origin and extent of the disease. This narrative review summarizes current therapeutic approaches, indications, and limitations, based on articles indexed in PubMed over the last five years. Management depends on the tumor origin, extent of disease (Peritoneal Cancer Index - PCI), feasibility of complete cytoreduction, and the patient's clinical status. Cytoreductive surgery (CRS) combined with hyperthermic intraperitoneal chemotherapy (HIPEC) is an established treatment for pseudomyxoma peritonei and peritoneal mesothelioma but remains controversial for colorectal and gastric malignancies. In gastric cancer with peritoneal metastasis, normothermic intraperitoneal chemotherapy (NIPS) combined with systemic treatment has demonstrated improved overall survival and higher rates of negative cytology conversion. In platinum-resistant ovarian cancer with high FRα expression, mirvetuximab soravtansine showed survival benefits and objective response rates superior to conventional chemotherapy. Treatment must be individualized, and further evidence is essential to define optimized strategies integrating surgery, intraperitoneal therapies, and immunotherapy.

COMPREHENSIVE TREATMENT OF LOCALLY ADVANCED AND METASTATIC GASTRIC CANCER: THE ROLE OF NEOADJUVANT INTRA-ARTERIAL CHEMOTHERAPY

Introduction: Stage III-IV gastric cancer (GC) is characterized by high rates of locoregional recurrence and distant metastasis, resulting in a poor prognosis despite modern systemic chemotherapy regimens. Although triple- and quadruple-drug perioperative regimens have improved outcomes, a significant proportion of pat...

Peritoneal Carcinomatosis as A Remaining Challenge of Abdominal Oncology: Contemporary Possibilities of Combined Modality Treatment

The present paper reviews the literature data on the current state of peritoneal carcinomatosis treatment. The international oncological associations of NSSN, ESMO, and AJCCA recommend chemotherapy to treat peritoneal carcinomatosis as a distant metastasis of malignant tumors. In recent years, the issues of combined mo...

Treatment Outcomes for Gastric Cancer With Positive Peritoneal Lavage Cytology or Localized Peritoneal Metastasis.

Importance In the era of modern systemic chemotherapy, the optimal treatment strategy for gastric cancer with positive peritoneal lavage cytology (CY1) and/or localized peritoneal metastasis (P1a) remains controversial. Objective To explore the optimal treatment strategy for patients with gastric cancer with CY1 and/...

Systemic Chemotherapy in Patients with Synchronous Peritoneal Metastasis from Gastric and Esophagogastric Junction Cancers Has No Uniform Effect on the Peritoneal Cancer Index.

BACKGROUND The prognosis of patients with peritoneal metastasis (PM) from gastric cancer or adenocarcinoma of the esophagogastric junction (EGJ) remains poor. Diagnostic laparoscopy plays a key role in evaluating the extent of PM, facilitating accurate Peritoneal Cancer Index (PCI) assessment. However, the extent to wh...

Advances in therapeutic approaches to gastric cancer

Abstract Gastric cancer remains a leading cause of cancer‐related mortality worldwide, with nearly 1 million new cases diagnosed annually. Despite innovations in diagnosis, most patients still present at advanced stages, contributing to a poor prognosis. For early gastric cancer, endoscopic resection or minimally invas...
