TL;DR

Stress phase entropy derived from ECG-gated SPECT MPI was a strong long-term predictor of MCEs in Japanese patients with CAD and it is considered to be the most useful left ventricular dyssynchrony index to improve risk stratification over prediction by perfusion and LVEF assessment alone.

Abstract

Objective This retrospective study aimed to investigate the long-term prognostic value and risk stratification in Japanese patients with known or suspected coronary artery disease (CAD) using poststress phase entropy, an index of left ventricular (LV) dyssynchrony. Methods The patients were followed up for six years to confirm their prognosis. The composite endpoint was the onset of major cardiac events (MCEs), including cardiac death, non-fatal myocardial infarction, and severe heart failure requiring hospitalization. Poststress phase standard deviation (SD), bandwidth, and entropy were calculated using a phase analysis with the Heart Risk View-F software program.Patients The study included 2,421 patients with known or suspected CAD who underwent rest 201Tl and stress 99 mTc-tetrofosmin electrocardiogram-gated (ECG)-gated single-photon emission computed tomography (SPECT) myocardial perfusion imaging (MPI).Results During the follow-up, 293 patients (13%) experienced MCEs. A multivariate analysis showed that age, the summed difference score, estimated glomerular filtration rate, stress LV ejection fraction (EF), and stress entropy were independent predictors of MCEs. A Kaplan-Meier analysis showed significant risk stratification between each group in the stress phase entropy tertiles. The global chi-squared value predicting MCE onset was significantly improved in the model with the summed stress score and stress LVEF plus entropy, but it was comparable in the model with added bandwidth or phase SD.Conclusion Stress phase entropy derived from ECG-gated SPECT MPI was a strong long-term predictor of MCEs in Japanese patients with CAD (known or suspected) and it is considered to be the most useful left ventricular dyssynchrony index to improve risk stratification over prediction by perfusion and LVEF assessment alone.

Long-term prognosis of coronary artery calcium scoring on top of cardiac SPECT in stable symptomatic patients.

BACKGROUND The coronary artery calcium score (CACS) has strong prognostic value in stable chest pain patients. However, studies with follow-up beyond 5 years are scarce. We studied the long-term prognosis of CACS beyond pre-test probability (PTP) and single-photon emission computed tomography (SPECT) results for major...

Prognostic value of soluble ST2 in cardiac magnetic resonance defined myocardial infarction with non-obstructive coronary arteries

Background Cardiac magnetic resonance (CMR) plays an essential role in identifying the underlying etiology of myocardial infarction with non-obstructive coronary arteries (MINOCA). However, risk stratification in patients with CMR-confirmed MINOCA remains insufficiently defined. Soluble suppression of tumorigenicity 2...

Outcome Stratification in Non-ST-Segment Elevation Myocardial Infarction: Standardized Assessment of Left Ventricular Global Longitudinal Strain

Background: Left ventricular global longitudinal strain (LVGLS) is a sensitive index of left ventricular systolic function. However, the prognostic value of this metric in patients with acute non-ST-segment elevation myocardial infarction (NSTEMI) has not been fully established. Methods: We retrospectively enrolled pat...

Evaluation of Clinical SYNTAX Score in Predicting New-Onset Atrial Fibrillation and Long-Term Prognosis in Patients Hospitalized with Non-ST Segment Elevation Myocardial Infarction

New-onset atrial fibrillation (NOAF) is a frequent complication among patients with non-ST-elevation myocardial infarction (NSTEMI) and is associated with adverse outcomes, making early identification of predictors essential for risk stratification and management. This retrospective cohort study evaluated the prognosti...

Prognostic value of D-SPECT-derived myocardial flow reserve in detecting coronary microvascular disease and predicting outcomes after complete revascularization: a two-center pilot study.

In this two-center cohort of patients with complete revascularization after PCI, D-SPECT-derived CMD is associated with a 2-fold increased risk of future MACE, independent of traditional risk factors and perfusion abnormalities.

Vasodilator stress perfusion CMR for outcome prediction in patients under 45 years with suspected CAD.

BACKGROUND Although the prognostic value of vasodilator stress cardiovascular magnetic resonance (CMR) is established, it has never been assessed in patients <45 years with suspected coronary artery disease. The aim was to assess the long-term prognostic value of stress CMR to predict mortality in a multicenter cohort...
