Abstract
Purpose To measure high contrast visual acuity (HCVA) when presbyopic soft contact lens (CL) wearers, requiring a spectacle addition from +0.75 to +1.75 dioptres were fit bilaterally with delefilcon A daily disposable multifocal (DT1 MF) CLs and senofilcon A daily disposable multifocal (AOM MF) CLs. Methods In this prospective, randomized, double-masked, cross-over, non-inferiority study, presbyopic CL wearers wore two types of multifocal CLs for 1 hour each. HCVA was measured monocularly and binocularly at 6m, 66cm and 40cm under photopic and mesopic conditions. Participants rated subjective visual clarity (0–100 scale, 100=very clear) after each HCVA assessment. Pupil size was assessed under photopic and mesopic conditions. Results Twenty-eight participants (48.6 ± 5.3 years) completed the study. DT1 MF was non-inferior to AOM MF in HCVA at 6m, 66cm and 40cm in photopic and mesopic conditions for the dominant and non-dominant eye, and binocularly. There were no statistically significant differences between lenses for subjective visual clarity ratings for any of the viewing distances, whether evaluated for photopic or mesopic conditions for the dominant or non-dominant eye or binocularly. Mean pupil size in the dominant eye was 3.6 ± 0.5mm (2.6 to 4.5mm) and 5.1 ± 0.8mm (2.8 to 6.1mm) in photopic and mesopic conditions respectively. HCVA under photopic or mesopic illumination at 6m, 66cm or 40cm was not associated with pupil size (Pearson correlation, all p>0.05). Conclusion Both lenses provided good vision across a variety of distances at high and low illumination. DT1 MF was non-inferior to AOM MF in HCVA under photopic and mesopic illumination at distance, intermediate and near. HCVA at all distances with both study lenses was independent of pupil size. Despite their different design, both study lenses performed equally well and provide a suitable vision correction for presbyopic CL wearers, irrespective of pupil size.
The customized multifocal corneo-scleral CLs with SA control evaluated can provide good distance and intermediate visual performance in presbyopic patients, but limited near visual performance and symptom tolerance, supporting the value of individualized optical design.

It is suggested that when low myopic astigmatism is corrected using spherical-equivalent soft contact lenses, certain binocular vision parameters may demonstrate acute changes and careful binocular vision assessment may therefore be beneficial when prescribing simplified spherical correction in astigmatic patients.

PURPOSE
Prolonged digital device use can result in digital eye strain which may be exacerbated by uncorrected astigmatism. This study explored how sustained smartphone reading impacts on visual function and symptoms when astigmatism is corrected with disposable soft toric contact lenses.
METHODS
30 adult contact lens wearers (mean age 23 ± 4 years, mean astigmatism 1.12 ± 0.35 D) habitually using digital devices for ≥10 h/day were fitted with Delefilcon A daily disposable toric contact lenses for 14-days. High and low contrast near visual acuity (VA) were assessed before and after a prolonged smartphone reading task on day 1 and day 14 of lens wear. The severity of digital eye strain symptoms were reported using a 0-100 visual analogue scale following the reading task. Contact lens dry eye symptoms were assessed using the 8-item Contact Lens Dry Eye Questionnaire (CLDEQ-8).
RESULTS
At baseline, the mean high and low contrast near VAs were - 0.08 ± 0.05 and + 0.04 ± 0.08 logMAR, respectively, which did not decline significantly following the prolonged reading task, or after 14 days of lens wear. The average reading speed was not significantly different between day 1 and day 14 of lens wear (251 ± 85 words-per-minute vs 252 ± 80 words-per-minute, p = 0.47). Digital eyestrain symptoms following the reading task (mean of all symptom scores: 13 ± 21) remained stable over the 14-days of lens wear (all p > 0.05), and > 75% of the reported symptoms were ranked as <15/100 in severity. The mean CLDEQ-8 score was 15.2 ± 8.8.
CONCLUSION
The correction of astigmatism with disposable toric contact lenses provided stable functional visual performance, with minimal digital eye strain during a prolonged smartphone reading task. However, over half of participants, reported significant dryness/discomfort (CLDEQ-8, ≥12) during 2-weeks of wear, highlighting a disconnect between short-term digital task symptoms and overall daily lens comfort in participants habitually using digital devices for ≥10 h/day.

To evaluate visual performance, contrast sensitivity (CS), optical quality, and subjective satisfaction of Vivity extended depth-of-focus (EDOF) premium intraocular lens (pIOL) in early keratoconus (KC). In this prospective, interventional, single-center clinical study, ten patients with mild bilateral KC and cataract received AcrySof IQ Vivity pIOL. Pre- and postoperative assessments included monocular and binocular uncorrected and distance-corrected visual acuities at 4 m (UDVA, DCVA), uncorrected and distance-corrected intermediate and near visual acuities at 66 cm (UI66VA, DCI66VA) and 40 cm (UNVA, DCNVA), defocus curve, CS, wavefront aberrometry (RMS, PSF, Strehl ratio), halometry, and subjective outcomes. At 3 months, mean ± SD monocular DCVA in the dominant eyes (primary endpoint) was 0.06 ± 0.13 logMAR. A statistically significant improvement was observed between preoperative and postoperative monocular and binocular UDVA, UNVA, DCNVA (p < 0.001), and DCVA (p = 0.03). Binocular DCI66VA and UI66VA were 0.10 ± 0.15 and 0.14 ± 0.16 logMAR, respectively. The defocus curve was ≤ 0.20 logMAR from + 0.50 D to – 2.00 D. CS peaked under scotopic conditions at 6 cpd, total ocular RMS improved from 2.15 ± 1.98 to 1.09 ± 0.77 (p = 0.09). NEI-RQL-42 scores indicated favourable patient-reported outcomes, particularly for glare, clarity of vision, and satisfaction with correction. In this small prospective pilot study, bilateral implantation of a non-diffractive EDOF IOL was associated with favourable short-term visual outcomes in highly selected patients with stable early keratoconus. These preliminary findings suggest that early stable KC may not represent an absolute contraindication to non-diffractive EDOF IOL implantation, but larger comparative studies with longer follow-up are required.

SMILE with micro-monovision had an impact on accommodative function in the early postoperative stage in myopic patients with presbyopia, and patients exhibited good distance and near vision, maintained binocular visual functions, and reported high subjective satisfaction.
