Visual and accommodative outcomes after femtosecond LASIK with and without monovision in patients over 40 years.

AIM To evaluate changes in visual acuity and accommodation function following femtosecond laser-assisted in situ keratomileusis (FS-LASIK) combined with monovision in patients aged over 40y. METHODS This prospective, non-randomized cohort study included patients who were divided into an FS-LASIK binocular group (full correction) and an FS-LASIK monovision group. The binocular group consisted of Group 1 (15 patients) with simple myopia and Group 2 (12 patients) with myopia and presbyopia. The monovision group (Group 3) included 13 patients with myopia and presbyopia. Parameters assessed included the uncorrected distance visual acuity (UCVA), the uncorrected near visual acuity (UNVA), contrast sensitivity (CS), binocular cross-cylinder (BCC), accommodative amplitude (AMP), positive relative accommodation (PRA), negative relative accommodation (NRA), and accommodative facility (AF). Patient satisfaction was evaluated at 3mo postoperatively. RESULTS A total of 40 patients (80 eyes) were enrolled (mean age 43.63±3.666y; 12 males). At 3mo after FS-LASIK, UCVA improved significantly in all patients (P<0.001), while UNVA remained comparable to preoperative levels (P>0.05). BCC in Group 3 decreased by 0.30±0.43 compared to preoperative levels, showing a statistically significant difference (P=0.037). The AMP in Group 3 showed an increasing trend with greater improvement in the nondominant eye than in the dominant eye (P>0.05). Patient satisfaction was high in all groups. CONCLUSION FS-LASIK combined with monovision is an effective treatment for patients aged over 40y with myopia and presbyopia, which not only improves visual acuity but also achieves high patient satisfaction and improves postoperative BCC.

Clinical outcomes of combined presbyopia and astigmatism correction following hydrophilic trifocal toric intraocular lens implantation in cataract patients: a prospective clinical study

Presbyopia and corneal astigmatism frequently coexist in cataract patients, reducing visual quality and increasing spectacle dependence. Toric trifocal intraocular lenses (IOLs) aim to address both conditions simultaneously. This prospective study evaluated the visual performance, quality, patient satisfaction, and safety profile of the Liberty 677MTY toric IOL, and classified its functional profile based on distance-corrected monocular defocus curve analysis. This prospective, single-center, non-comparative clinical investigation included 28 eligible cataract patients with corneal astigmatism (1.00–6.00 D) who underwent bilateral implantation of the Liberty 677MTY trifocal toric IOL. Uncorrected and distance-corrected visual acuities at all distances, subjective refraction, IOL rotational stability, defocus curves, contrast sensitivity, patient-reported visual function, and safety were reported during a 12-month follow-up period. The Liberty 677MTY IOL demonstrated significant improvements (p < 0.05) in uncorrected and distance-corrected visual acuities and subjective refractive cylinder, with stability over time. IOL alignment was stable through follow-up, with mean signed and absolute rotations of < 2° and < 5°, respectively. Binocular visual acuity defocus curves showed visual acuities better than 0.2 logMAR across the range of + 1.00 to ‑3.50 D. Monocular visual acuity defocus analysis confirmed full range of field smooth (FRoF-Sm) classification of the IOL. Contrast sensitivity defocus curve met acceptance criteria. Based on patient-reported outcomes 89% rated satisfaction as high, and 83% reported mild or no difficulty with visual disturbances. At 12 months, spectacle independence was achieved in 96.3% of patients for far, 81.5% for intermediate, and 92.6% for near vision. Safety analysis revealed one Nd: YAG-treated PCO case (1.61%) and no other IOL-related adverse events. This study provides a detailed evaluation of the Liberty 677MTY trifocal toric IOL, integrating real-life binocular visual experience with comprehensive monocular assessments. The lens demonstrated predictable refractive outcomes, excellent rotational stability, and consistently high-quality vision across all distances, with strong patient satisfaction. Classified as FRoF-Sm by monocular defocus curve analysis, the lens offers continuous functional vision. By integrating monocular and binocular perspectives, these findings offer a clearer understanding of the lens’s performance, supporting its use as a safe and effective solution for achieving spectacle independence and enhancing postoperative quality of life.

Clinical Outcomes after Bilateral Mix-and-Match Implantation of Extended Depth of Focus and Trifocal Intraocular Lenses

Purpose To evaluate the clinical outcomes after bilateral mix-and-match implantation of extended depth of focus (EDOF) and trifocal intraocular lenses (IOLs). Methods A total of 134 eyes of 67 patients who had met all inclusion and exclusion criteria and underwent bilateral cataract surgery were included. The Symfony or Symfony toric EDOF IOL (Johnson & Johnson Vision Care Inc.) was implanted in the dominant eye, and the PanOptix or PanOptix toric trifocal IOL (Alcon Laboratories Inc.) in the nondominant eye. The uncorrected distance visual acuity (UDVA), uncorrected intermediate visual acuity (UIVA), uncorrected near visual acuity (UNVA), corrected distance visual acuity (CDVA), defocus curve, contrast sensitivity, and satisfaction questionnaire were obtained preoperatively and at 1, 6, and 12 months after surgery. Results At 12 months after surgery, the mean UDVA (logMAR) was 0.03 ± 0.06 in the EDOF IOL-implanted eyes, 0.07 ± 0.11 in the trifocal IOL-implanted eyes, and 0.02 ± 0.06 in both eyes. The mean UIVA was 0.08 ± 0.08 in EDOF IOL-implanted eyes, 0.09 ± 0.11 in trifocal IOL-implanted eyes, and 0.03 ± 0.08 in both eyes. Mean UNVA was 0.51 ± 0.26 in EDOF IOL-implanted eyes, 0.22 ± 0.16 in trifocal IOL-implanted eyes, and 0.17 ± 0.12 in both eyes. Overall satisfaction rates were 43.3%, 41.1%, and 38.5% at 1, 6, and 12 months, respectively, while more than 70% of patients consistently reported that they would recommend the surgery to others. The most common causes of dissatisfaction were dry eye symptoms and reduced near vision. Conclusions Bilateral mix-and-match implantation of EDOF and trifocal IOL implantation showed good visual outcomes for distance, intermediate, and near vision until 12 months. Most patients satisfied with visual quality, but more dissatisfied with dry eye disease and poor near vision than photopic phenomena like glare and halos.
