Vaccines, Drug Development, and Novel Therapeutic Strategies for Hepatitis: Current Advances and Future Perspectives

Despite remarkable progress in prevention and treatment, hepatitis is still one of the main drivers of chronic liver disease, liver cirrhosis and hepatocellular carcinoma and is a big public health problem worldwide. The hepatitis viruses A, B, C, D and E are responsible for the occurrence of viral hepatitis in millions of people and are a significant cause of morbidity and mortality worldwide. Vaccination has been found to be very effective in preventing hepatitis A and B infections, and vaccination is not licensed for use in hepatitis C or used widely enough in hepatitis E. At the same time, there has been great advancement in antiviral drug development, including nucleotide analogues, pegylated interferons and direct-acting antivirals, which have greatly improved clinical outcomes, especially in the treatment of chronic hepatitis B and C. However, there are several issues that need to be addressed including the high cost of healthcare, accessibility of treatment, partial functional cure, persistent viral reservoirs, and antiviral resistance, which requires innovative therapeutic strategies. New technologies, such as therapeutic vaccines, gene editing via CRISPR/Cas systems, RNA interference, monoclonal antibodies, immune modulators, and host-targeted treatment and drug delivery via nanotechnology have the potential to improve antiviral efficacy and sustained viral eradication. Additionally, advancements in AI, computational drug discovery, pharmacogenomics, and precision medicine are driving the discovery of new drug targets, optimization of drug interventions, and the personalized treatment strategies. This review will provide a comprehensive overview of the current status of hepatitis vaccines, recent developments in antiviral drug discovery, and new developments in therapeutic innovations. It also highlights the current clinical trials, translational research, key challenges and future directions towards the global target of hepatitis elimination. Together, these advances demonstrate the exciting possibilities for multidisciplinary care in the prevention, treatment, and long-term clinical outcomes of patients with viral hepatitis.
