TL;DR

ADI and DCI identify complementary socioeconomic disparities in nephrolithiasis, more strongly associated with categorical urine abnormalities and stone composition, whereas DCI correlated more closely with continuous urine chemistries.

Abstract

Objective

To compare the Area Deprivation Index (ADI) and Distressed Communities Index (DCI) in identifying socioeconomic disparities in 24-hour urine (24hU) parameters and stone composition among patients undergoing percutaneous nephrolithotomy (PCNL).

Methods

Retrospectively, 1859 adults who underwent PCNL between 2017 and 2022 and completed at least one 24hU collection were included. Patients were assigned ADI and DCI percentiles based on residential location. Demographics, urine chemistries, categorical urine abnormalities, and stone composition were compared across index quartiles. Multivariable logistic regression adjusted for age, sex, race, and body mass index assessed associations between socioeconomic quartiles and urinary abnormalities or stone type. Predictive performance of ADI- and DCI-based models was compared using discrimination and non-nested model testing.

Results

Greater socioeconomic deprivation was associated with younger age, higher body mass index, and lower completion of metabolic testing. Higher ADI quartiles were associated with lower urine volume, lower urinary potassium, increased odds of low urine volume, and hypocitraturia. DCI quartiles demonstrated similar associations with urine volume but were additionally associated with urinary oxalate and uric acid, while showing no significant association with hypocitraturia. ADI quartile 4 was associated with lower odds of calcium oxalate stones and higher odds of struvite and calcium phosphate stones, whereas DCI showed weaker associations with stone composition. For outcomes significantly associated with both indices, neither demonstrated superior predictive performance.

Conclusions

ADI and DCI identify complementary socioeconomic disparities in nephrolithiasis. ADI was more strongly associated with categorical urine abnormalities and stone composition, whereas DCI correlated more closely with continuous urine chemistries.

Association between the Zhejiang University index and kidney stone risks in US adults: evidence from NHANES 2007-2018.

BACKGROUND Kidney stones carry a substantial population health burden and are demonstrated to be increasingly associated with metabolic dysregulation. We examined whether the Zhejiang University (ZJU) index, a composite metabolic parameter derived from routine clinical data, is associated with kidney stone history in U...

The association between sociodemographic and clinical factors and outcomes among inpatients with community-acquired pneumonia.

BACKGROUND Community-acquired pneumonia (CAP) is a leading cause of hospitalization and mortality. The influence of sociodemographic characteristics on adverse outcomes in adults hospitalized with CAP remains poorly understood, particularly for patients younger than 65. OBJECTIVES To evaluate associations between bas...

Sex-based Disparities in In-Hospital Outcomes Among Patients Hospitalized with Hyperthyroidism: A National Inpatient Sample Analysis (2016-2022).

OBJECTIVE Hyperthyroidism is more common in women, but hospitalized men may have worse outcomes. National U.S. data are limited. we aimed to compare in-hospital mortality, length of stay, and total charges by sex among adults hospitalized with hyperthyroidism. METHODS We analyzed 2016-2022 NIS adult hospitalizations...

Impact of Socioeconomic Status on Early Gastric Cancer Outcomes and Treatment Disparity: a SEER analysis

The influence of socioeconomic determinants of health on early gastric adenocarcinoma (GAC) remains understudied. We evaluated the association between median household income (MHI), treatment patterns, and survival in T1aN0M0 GAC. Using the SEER-17 Research Plus database (2004–2021), we identified 3,792 patients with T...
