Spectrum of uterine malignancies and their oncological outcomes: a single centre experience

Abstract

Background: Endometrial carcinoma is a significant contributor to the global female cancer burden. Its incidence in India is rising because of increasing life expectancy and metabolic risk factors, although Indian outcome data remain limited. This study aimed to evaluate the spectrum of uterine malignancies and assess their oncological outcomes in an Indian cohort. Methods: This prospective observational study was conducted at a tertiary care centre over a five-year period (2020–2025). Seventy (70) women were included following histopathological confirmation of uterine malignancy. Data on clinical presentation, histological subtype, staging, treatment, and outcomes were recorded and analysed using descriptive statistics. Results: The mean age was in the sixth decade, with 88.6% of patients being postmenopausal. Endometrioid adenocarcinoma was the most common histological subtype (75.7%), followed by serous carcinoma (7.1%), carcinosarcoma (4.3%), and clear cell carcinoma (4.3%). Most patients presented at FIGO Stage I (61.5%) and underwent primary surgical management. Adjuvant therapy was administered in 32.9% of cases based on stage and risk factors. At a median follow-up, 72.9% of patients were alive and recurrence was observed in only 1.4%. Binary logistic regression showed that none of the individual pathological factor’s tumour grade, histology, LVSI, myometrial invasion, or cervical stromal involvement significantly predicted lymph node involvement. Conclusion: Endometrioid carcinoma remains the predominant uterine malignancy in Indian women, typically presenting in postmenopausal women at an early stage with favourable outcomes. Given the inability of conventional pathological risk factors to reliably predict nodal involvement, comprehensive surgical staging using sentinel lymph node mapping or systematic lymphadenectomy should be considered in all cases, irrespective of initial stage.

Dual primary malignancies are no longer a rarity: A single centre experience from eastern India

Advancements in cancer therapies have improved long-term survival rates, contributing to an increased incidence of second primary malignancies. This study aims to describe the clinical characteristics, tumour profiles, and treatment patterns of patients diagnosed with dual primary malignancies. A retrospective review of prospectively maintained hospital records of patients diagnosed with histologically confirmed dual primary malignancies was performed from January 2023 to December 2024. Classification into synchronous and metachronous tumours was done as per the criteria by Warren and Gates. Demographic data, tumour site, chronology, and treatment details were collated for analysis. In this study, a total of 21 patients were found to have dual primary malignancies, which included 12 (57.1%) males and 9 (42.9%) females. Out of these, 8 (38.1%) were synchronous, and 13 (61.9%) were metachronous lesions. Common sites of first primary malignancy included breast, colon, head and neck, rectum, and prostate, whereas common sites of second primary malignancies included the thyroid, especially papillary carcinoma, followed by breast, gastrointestinal tract, and prostate. Six (28.6%) patients had breast malignancy, where it was the primary malignancy in 4 cases and a secondary malignancy in 2 cases. One patient with bilateral breast carcinoma is also included in this category. With the improving survival after cancer treatment, dual primary malignancies are being detected with increasing frequency. The key to improving outcomes in this growing subset of oncology patients lies in early identification by vigilant follow-up, appropriate screening, and management strategies tailored to the individual patient.

Optimizing care for low-grade serous ovarian carcinoma: insights from a tertiary care centre

Background: Low-grade serous ovarian carcinoma (LGSC) is an uncommon epithelial ovarian malignancy with distinct tumour biology, relatively indolent behaviour, frequent advanced-stage presentation and limited sensitivity to conventional chemotherapy. This study evaluated the clinicopathological profile, treatment pattern, perioperative outcomes and survival of patients with LGSC managed at a tertiary cancer centre. Methods: A retrospective review was conducted of 23 patients with histologically confirmed LGSC treated between 2014 and 2025. Demographic characteristics, clinical presentation, tumour markers, FIGO stage, disease distribution, chemotherapy response, surgical approach, completeness of cytoreduction, postoperative morbidity, recurrence and survival outcomes were analyzed descriptively. Results: The median age was 47 years. Abdominal pain was the most common presenting symptom, reported in 17 patients (73.91%). Serum CA-125 was elevated in 19 patients (82.6%), with a median value of 89 U/ml. FIGO stage III disease was present in 18 patients (78.26%). Nine patients received neoadjuvant chemotherapy; objective response was observed in only 2 patients (22.2%), while 5 had stable disease and 2 progressed. Primary cytoreductive surgery was performed in 12 patients, interval cytoreduction in 9 and staging laparotomy in 2. Complete cytoreduction was achieved in 19 patients (82.6%). Grade III morbidity occurred in 21.7%, with no Grade IV/V complications or 30-day mortality. At 36-month median follow-up, recurrence occurred in 6 patients; 3 years overall survival was 75% and disease-free survival was 66%. Conclusions: LGSC commonly presents with advanced peritoneal disease and poor chemotherapy response. Complete cytoreduction remains the key therapeutic goal.

Surgery of gallbladder carcinoma: current standards validated on a European cohort

Gallbladder cancer (GBC) is a highly aggressive malignancy with a poor prognosis and varied global incidence. Despite surgical resection being the primary treatment, optimal approaches remain heterogeneous and debated due to limited high-quality evidence. This study aims to validate current surgical recommendations in a European real-world cohort. We retrospectively analyzed data from 150 GBC patients who underwent surgery at the University Hospital of Essen, Germany (2008–2023). Patient and tumor characteristics (UICC/TNM stage, incidental diagnosis, surgical approach), perioperative outcomes (mortality, morbidity, length of hospital stay), and overall survival (OS) were recorded. Congruence with established surgical recommendations was assessed, and its impact on OS analyzed using multivariate regression. Our cohort, predominantly female (60%) with a mean age of 64.9 years, included 42% of incidental cases. Overall mortality was 6.3%, and morbidity was 24.6%. Curative intent resection was performed in 57% of patients. Major resections and extrahepatic bile duct resections were associated with higher perioperative risk. R0 resection, regardless of extent, yielded better OS than palliative treatment (21–27 months vs. 8.4 months p  = 0.001). OS correlated with UICC stage (hazard ratio [HR] 2.23, p  = 0.001). Importantly, noncongruence with surgical recommendations was significantly associated with worse OS (HR 5.1, p  = 0.006). Our findings validate current recommendations for GBC surgery, emphasizing the importance of adherence to achieve optimal outcomes. Despite high morbidity in advanced resections, R0 status remains crucial. These data from a European cohort contribute to bolstering evidence for current surgical standards in GBC management.
