Obturation for Patient-Centered Endodontic Outcomes: Tips and Details That Make a Difference in the Outcome

Sep 2026 · Macedonian Medical Electronic Journal · 0 citations

Abstract

INTRODUCTION: Premixed bioceramic sealers are increasingly incorporated into daily endodontic practice, offering simplified obturation protocols and new approaches to root canal filling. However, questions remain regarding their long-term clinical effectiveness, particularly in teeth affected by apical periodontitis. Achieving a reliable seal through appropriate obturation is essential for predictable healing and successful endodontic outcomes.

Aim

The aim of this lecture is to discuss the clinical impact of premixed bioceramic sealers and to identify specific technical details and clinical protocols that may influence treatment outcomes. Particular attention will be given to whether obturation with bioceramic sealers provides reliable clinical results and whether simplified techniques, such as the single-cone method, can achieve predictable outcomes. CONTENT: The lecture will focus on hydraulic calcium silicate-based sealers and their application in non-surgical endodontic treatment, with particular emphasis on cold hydraulic condensation (CHC) using a bioceramic sealer. Factors that may affect the outcome of teeth with apical periodontitis will be discussed, including case selection, canal preparation, obturation technique, sealer application, and technical details that can influence the quality and predictability of the final result. Clinical data from a retrospective study with a 5-year observation period will be presented. The study included 637 teeth with symptomatic apical periodontitis treated by a single experienced operator using CHC with CeraSeal bioceramic sealer..

Conclusion

Understanding the biological and technical principles of hydraulic obturation, together with careful attention to clinical protocols and procedural details, is essential for achieving predictable endodontic outcomes. Cold hydraulic condensation with bioceramic sealers may represent a clinically relevant approach to obturation; however, its effectiveness depends not only on the material itself but also on appropriate case selection, treatment execution, and adherence to sound endodontic principles. The lecture will highlight practical tips and details that may contribute to achieving high success rates and improved patient-centered outcomes.
