It is suggested that short‐term perioperative sotatercept can serve as an effective acute rescue therapy to safeguard high‐risk surgical interventions in critical pediatric PAH.
Abstract
ABSTRACT The reverse Potts shunt (RPS) is a promising palliative intervention for pediatric patients with drug‐refractory severe pulmonary arterial hypertension (PAH), yet it carries substantial risks of perioperative mortality and hemodynamic instability. We report two high‐risk pediatric female patients who developed acute hemodynamic instability and recurrent pulmonary hypertensive crises following successful percutaneous RPS creation. Despite maximal targeted triple therapy and vasoactive support, no sustained improvement was observed. Following multidisciplinary consensus, a single salvage dose of sotatercept (0.3 mg/kg) was administered. Both patients demonstrated rapid, hemodynamic stabilization within 48 h, enabling the swift withdrawal of vasoactive support and inhaled nitric oxide within 1 week. Follow‐up echocardiography revealed significant recovery of right ventricular function, and both patients were successfully weaned off treprostinil. These findings suggest that short‐term perioperative sotatercept can serve as an effective acute rescue therapy to safeguard high‐risk surgical interventions in critical pediatric PAH.
Pediatric pulmonary arterial hypertension carries a poor prognosis. Reversed Potts shunt has been proposed as a bridge from medical therapy to lung transplantation, yet evidence supporting its ability to preserve right ventricular function and improve symptoms in this population remains limited.

ABSTRACT We report the case of an 18‐year‐old female with progressive, end‐stage pulmonary arterial hypertension (PAH) despite receiving maximal triple combination therapy. The patient had a history of patent ductus arteriosus (PDA) closure at 15 months of age. Due to a pulmonary hypertensive crisis during anesthesia i...
Potts shunt represents a strategy for children with refractory pulmonary arterial hypertension (PAH). Procedural timing remains undefined, and interventions may be associated with perioperative mortality and limited clinical benefit. Evidence integrating advanced imaging, multidisciplinary decision-making, and ri...

Sotatercept is a drug approved for pulmonary arterial hypertension (PAH) that improves mortality, right ventricular function, functional class, NT-proBNP, and 6-minute walk distance. However, no clinical trials support its use in patients with pulmonary hypertension (PH) and interstitial lung disease (ILD), and respira...

Drug-refractory atrial tachyarrhythmia is an important precipitating factor for recurrent heart failure decompensation in patients with Eisenmenger syndrome (ES). Although percutaneous pulsed field ablation (PFA) represents a potential therapeutic option for arrhythmia management, its application in this populati...
