Sociodemographic and Clinical Profile of Patients Presenting with Breast Cancer at a Tertiary Care Hospital: A Retrospective Study

Abstract

Introduction: Breast cancer, a type of cancer that originates in the breast tissue, is considered as an important public health problem in all the countries, especially in the developing countries. It is the commonest malignancy affecting women worldwide and is the leading cause of cancer related deaths. Aim: The aim of this study was to analyze the sociodemographic and clinical profile of confirmed breast cancer patients. Materials and Methods: This was a retrospective observational study done at State Cancer Institute, Guwahati. Data of Breast Cancer patients registered during the five-year study period (2018- 2022) were collected from Hospital Based Cancer Registry (HBCR) and the institutional ICMR-NCDIR (National Centre for Disease Informatics and Research Department). The parameters taken for study were age, sex, marital status, religion, education level, residence (urban/rural), stage at time of presentation, primary histology and morphology, histological type, laterality (right/left), the quadrant of the breast involved and metastasis. Results: A total of 1563 cases were analyzed of which 1529 were female (97.83%) and 34 were male (2.17%) with a male female ratio of 1:45. The age of patients ranged between 16 to 84 years and majority were urban based. Carcinoma on right breast was more common, upper outer quadrant of the breast was the most commonly affected site. Most of the patients presented in Stage II (41%) and 15% of the patients were in Stage IV with metastasis. Bony metastasis was the commonest (47%) followed by lung metastasis (27%). NOS was the most common Primary Histology –Morphology. Conclusion: The pattern found in our study was similar to other Indian studies where breast cancer is common among younger age groups in contrast to western countries where breast cancer is seen in post-menopausal women. Most of the patients presented in the advanced stage. The results show a trend in rise of prevalence in younger age groups, more males affected than previous studies and presentation in the advanced stage. There is urgent need for awareness programs, population screening for early diagnosis of breast cancer which will improve the overall outcome of management of breast cancer.

Clinicopathological Spectrum, Molecular Subtypes, and Contemporary Treatment Patterns of Breast Cancer in Bangladesh

Introduction: Breast cancer is the most common malignancy among women worldwide and represents an increasing oncological burden in Bangladesh. Delayed presentation, limited screening facilities, and heterogeneity in tumor biology continue to influence treatment outcomes in low- and middle-income countries. This study aimed to evaluate the clinicopathological profile, molecular subtypes, treatment patterns, and short-term outcomes of breast cancer patients treated at a tertiary care teaching hospital in Bangladesh. Methodology: This retrospective observational study was conducted in the Department of Surgery, Anwar Khan Modern Medical College Hospital, Dhaka, Bangladesh. Histopathological confirmed breast cancer patients treated between January 2022 and June 2023 were included in the analysis. Results: A total of 43 patients were analyzed. The median tumor size was 3.0 cm (IQR: 2.0–4.0 cm), and the median symptom duration was 4.0 months (IQR: 2.0–6.0 months). ER/PR-positive, HER2- negative tumors constituted the most common molecular subtype. Tumor size greater than 2 cm was significantly associated with nodal positivity (OR 9.14, 95% CI 1.53–54.54; p=0.020). Tumor size demonstrated a significant positive correlation with the number of involved lymph nodes (rho=0.62, p<0.001). Symptom duration was also positively correlated with tumor size (rho=0.60, p<0.001). Conclusion: Breast cancer patients in Bangladesh continue to present with relatively advanced disease. Longer symptom duration was associated with larger tumor size, and larger tumor size was significantly associated with nodal positivity, highlighting the importance of early diagnosis, timely referral, and breast cancer awareness programs to improve clinical outcomes. Clinicopathological Profile of Lung Cancer Patients

Background: Lung cancer is a common malignancy and an important cause of cancer-related mortality. Institutional data from Eastern Nepal are limited. Materials and Methods: A descriptive hospital-based cross-sectional study was conducted in the Clinical Oncology Department of Nobel Medical College Teaching Hospital, Biratnagar, Nepal. Records of 50 confirmed lung cancer patients registered from January 2023 to December 2023 were reviewed. Data were retrieved from the oncology registry and hospital medical records and analyzed using descriptive statistics. Results: The mean age was 63.6 ± 11.3 years, with a median age of 68 years (range 27-79 years). Males and females were equally represented (25 each, 50.0%). Stage IV disease was observed in 30 (60.0%) patients and Stage III disease in 20 (40.0%) patients. Adenocarcinoma was the most common histological subtype (22, 44.0%), followed by squamous cell carcinoma (16, 32.0%) and small cell carcinoma (4, 8.0%). Smoking history was present in 35 (70.0%) patients. Conclusion: Lung cancer patients attending Nobel Medical College Teaching Hospital predominantly presented at advanced stages, and adenocarcinoma was the leading histological subtype.

A PROSPECTIVE STUDY OF BREAST CARCINOMA DEMOGRAPHY, STAGING, TREATMENT & ITS COMPLICATIONS IN TERTIARY CARE HOSPITAL

Background: Breast carcinoma is one of the most common malignancies among women and remains a major public health concern due to delayed presentation and disease-related morbidity. Early diagnosis, appropriate surgical management, and adjuvant therapy play an important role in improving patient outcomes.Methodology: This hospital-based prospective observational study was conducted among 80 female patients aged 18–65 years with histopathologically confirmed Stage I or II breast carcinoma. Demographic, clinical, reproductive, and treatment-related details were recorded usinga structured proforma. Clinical staging was done according to the TNM classification. Patients underwent Modified Radical Mastectomy (MRM) or Breast Conserving Surgery (BCS) as per institutional protocol. Histopathological findings, postoperative complications, and adjuvant therapies were analyzed using IBM SPSS Statistics.Results: The mean age of the study participants was 46.97 ± 10.24 years. Most patients were premenopausal (55.00%), multiparous (78.75%), had Stage II disease (92.50%), positive lymph nodes (80.00%), and invasive ductal carcinoma (90.00%), with the upper outer quadrant (63.75%) being the most common tumor site. MRM (77.50%) was performed more often than BCS (22.50%), mainly due to fear of recurrence (37.50%). Most patients had no postoperative complications, with seroma (7.50%) being the most common. All patients received radiotherapy, while 82.50% received chemotherapy and 63.75% received endocrine therapy.Conclusion: Breast carcinoma commonly affected middle-aged women and frequently presented at Stage II with nodal involvement. Invasive ductal carcinoma was the predominant histological subtype. The findings highlight the importance of early detection, timely surgical intervention, and multimodal adjuvant therapy in improving outcomes among breast carcinoma patients.

Cancer Distribution in an Oncology Outpatient Clinic in Eastern Türkiye: A 1-Year Retrospective Study

Objective: Cancer epidemiology may exhibit regional variations depending on geographical, environmental, and sociocultural factors. The aim of this study was to evaluate the sex distribution and cancer diagnoses of patients admitted to the Van province region and to compare the findings with current national and international literature. Methods: This study was designed as a single-center, retrospective, and descriptive study. A total of 728 patients who were admitted to the Medical Oncology Outpatient Clinic of Van Training and Research Hospital between January 1, 2024, and December 31, 2024, and whose malignancy diagnoses were histopathologically confirmed were included. Patient sex and cancer diagnoses were obtained from hospital records. Data were analyzed using descriptive statistics, and categorical variables were presented as numbers (n) and percentages (%). Results: Of the 728 patients included in the study, 387 (53.2%) were female and 341 (46.8%) were male. The most common cancer diagnoses were breast cancer (21.2%), gastric cancer (19.4%), esophageal cancer (10.7%), lung cancer (8.5%), and colorectal cancer (6.5%). Upper gastrointestinal system cancers, specifically gastric and esophageal malignancies, combined to constitute nearly one-third (30.1%) of the total patient population. Sex-specific analysis revealed a strong female predominance in breast cancer (97.4%), male predominance in gastric and lung cancers, and a nearly equal sex distribution in esophageal cancer. Conclusions: Female patients predominated slightly, and the incidence of gastric and esophageal cancers was significantly higher than reported in the literature. These findings suggest that regional environmental, dietary, and sociocultural factors play a critical role in shaping cancer distribution across the Eastern Anatolia region.

Clinicopathological Profile and Molecular Subtypes of Breast Cancer in Bangladeshi Women: A Tertiary-Care Surgical Population Study

Introduction: Breast cancer in Bangladeshi women often presents at a younger age with advanced stage. The variable molecular characteristics influencing prognosis and treatment response. Local clinicopathological and subtype data remain limited in tertiary surgical settings. The aim of this study is to evaluate the clinicopathological features and molecular subtypes of breast cancer among Bangladeshi women in a tertiary level. Methods: This prospective cohort study was conducted at Bangladesh Medical University (BMU), Dhaka, Bangladesh and East West Medical College Hospital (EWMCH), Dhaka, Bangladesh, from January 2019 to December 2022. A total of 147 histopathologically confirmed breast cancer patients were enrolled using purposive sampling. Demographic, clinical, histopathological, and immunohistochemical data Estrogen Receptor(ER), Progesterone Receptor(PR), Human Epidermal Growth Factor Receptor(HER2), Kiel 67(Ki-67) were recorded. Data were analyzed using SPSS version 23.0. Results: Most patients were £50 years (62.6%) and presented with stage II–III disease (72.8%). Invasive ductal carcinoma predominated (88.4%). Tumors >2 cm (71.4%) and nodal metastasis (64.6%) were common. ER positivity was 55.1%, PR positivity was 49.0%, HER2 overexpression 27.2%, and high Ki-67 58.5%. Luminal A was most frequent (32.0%), followed by luminal B (26.5%), triple-negative (23.1%), and HER2-enriched (18.4%). Aggressive subtypes were significantly associated with higher grade and larger tumors (p<0.05). Conclusion: Bangladeshi women with breast cancer frequently present at a younger age and advanced stage, with a substantial proportion of aggressive molecular subtypes. Clinicopathological Features and Management of Prostate Cancer at the University of Uyo Teaching Hospital, Nigeria

Aim: This study aimed to determine the clinicopathological features, comorbidities, management outcomes and hospital prevalence of prostate cancer (PCa) in a Nigerian teaching hospital. Study Design: This was a retrospective study of 210 patients diagnosed with prostate cancer. Place and Duration of Study: The study was conducted at the University of Uyo Teaching Hospital in Nigeria over a 10-year period, from 2013 to 2022. Methodology: Men aged 45-90 years with histologically diagnosed prostate cancer were included in the study. Data retrieved from patients' case files, ward/clinic records and operation registers included age, clinical features, investigation results, prostate-specific antigen, histological type, Gleason scores and treatment modalities. Clinicopathological characteristics and treatment outcomes were analysed. Hospital prevalence was calculated by dividing the number of PCa cases by the total number of adult male patients seen during the study period and was estimated per 100,000. Results: There were 139,072 males aged 40 years and above treated in the hospital during the study period, of whom 210 had a histological diagnosis of prostate cancer, giving a hospital prevalence of 151/100,000. The mean age was 64.66 ± 9.24 SD (range: 45-90 years), and the median age was 65 years. The mean initial PSA was 46.98 ± 34.15 SD. Most patients (96%) had adenocarcinoma, 2% had urothelial carcinoma and 2% had squamous cell carcinoma. Gleason scores of 5-7 were seen in 105 patients (50.0%) and scores of 8-10 in 91 patients (43.3%), while 14 patients (6.7%) had scores of 2-4. Advanced prostate cancer (TNM group stages III and IV) was present in 85.2% of patients, while 14.8% had group stages I and II disease. One hundred and ninety-one patients (90.9%) had androgen deprivation therapy with bilateral orchidectomy, luteinising hormone-releasing agonists, antiandrogens and oestrogens. Only ten patients (4.8%) with early localised lesions had radical retropubic prostatectomy or radical radiotherapy, while nine patients (4.3%) had no treatment. Approximately 70% of patients died within 5 years of diagnosis. The remaining patients were lost to follow-up. Conclusion: Patients with prostate cancer presented at a late stage, with locally advanced and metastatic disease. Most patients had moderately or poorly differentiated adenocarcinoma. Androgen deprivation therapy was the treatment modality for most patients. Prostate cancer screening and early detection are strongly advocated so that definitive therapy for localised lesions may reduce mortality.
