Abstract

Spontaneous intracranial hypotension is a frequently debilitating condition that is most commonly caused by loss of cerebrospinal fluid (CSF) from the spine. The most frequently occurring types of spontaneous spinal CSF leaks that cause spontaneous intracranial hypotension are ventral dural tears, lateral dural tears, and CSF venous fistulas, but other leak types are recognized including sacral dural tears or fistulas into vascular malformations. Localization and characterization of spinal CSF leaks with CT myelography facilitates subsequent targeted treatment. A tailored approach based on the suspected leak type as determined at MRI of the spine is necessary to maximize the likelihood of a diagnostic study. The authors focus primarily on advanced CT myelographic techniques for leaks from dural tears and CSF venous fistulas. The importance of optimal patient positioning, typical and atypical appearances of the different leak types, and artifacts that can mimic a CSF leak are considered. ©RSNA, 2026 Supplemental material is available for this article. Spontaneous spinal cerebrospinal fluid leak in very early pregnancy following frozen embryo transfer: A case report

Spontaneous intracranial hypotension, typically resulting from spontaneous leakage of spinal cerebrospinal fluid (CSF), is an uncommon and underrecognized condition, particularly during pregnancy. Diagnosis can be challenging when cranial imaging is normal, and presentations in very early pregnancy are sparsely reporte...

Application of cervical epidural blood patch for spontaneous cerebrospinal fluid leaks: Two case reports.

Spontaneous cerebrospinal fluid (CSF) leaks are uncommon but significant causes of orthostatic headaches, often resulting from dural injuries. Although lumbar epidural blood patches (EBPs) are commonly used to treat such leaks, cervical EBPs are rarely performed due to the higher risk of complications, including spinal...

Spontaneous Intracranial Hypotension With Prominent Dorsal Perimedullary Veins Mimicking a Spinal Dural Arteriovenous Fistula.

Enlarged and tortuous veins along the dorsal aspect of the spinal cord are traditionally associated with a spinal dural arteriovenous fistula (dAVF). We present six cases demonstrating prominent dorsal perimedullary spinal veins in patients without a dAVF, but rather with a spinal CSF leak resulting in spontaneous intr...

Endoscopic repair of spontaneous ventral spinal cerebrospinal fluid leaks after failed open intradural surgery, and vice versa.

OBJECTIVE Ventral spinal CSF leaks are a common cause of spontaneous intracranial hypotension (SIH) and may require surgical repair. Open intradural and endoscopic transforaminal extradural approaches may be considered for treatment of these ventral spinal lesions. The authors report their experience using complementar...

Salvage CT Myelography: Same-Session Repositioning to Localize Spinal CSF Leaks.

Localization of spinal CSF leaks may require multiple myelograms when the initially suspected CSF leak type is not identified. We describe a salvage CT myelography (CTM) technique in which the patient is repositioned during the same myelographic session to evaluate for an alternative CSF leak type. Eleven patients unde...
