Intrathecal clonidine with hyperbaric levobupivacaine improved block characteristics and prolonged postoperative analgesia in a dosedependent manner.

Abstract

Background:Intrathecal adjuvants are commonly used to improve the quality and duration of spinal anesthesia in lower limb orthopedic surgeries. Clonidine, an α2-adrenergic agonist, prolongs sensory and motor blockade and postoperative analgesia when combined with local anesthetics; however, the optimal intrathecal dose of clonidine with hyperbaric levobupivacaine remains uncertain. Objectives:To evaluate and compare the effects of intrathecal clonidine (15 μg and 30 μg) as an adjuvant to 0.5% hyperbaric levobupivacaine in patients undergoing lower limb orthopedic surgeries. Methods:This prospective, randomized, double-blind study included 99 ASA I–II patients undergoing elective lower limb orthopedic surgery under spinal anesthesia. Patients were randomly allocated into three groups (n=33 each): Group A received hyperbaric levobupivacaine with saline, Group B received levobupivacaine with clonidine 15 μg, and Group C received levobupivacaine with clonidine 30 μg. Sensory and motor block characteristics, duration of postoperative analgesia, hemodynamic parameters, postoperative Visual Analog Scale (VAS) scores, and adverse effects were evaluated and compared among the groups. Results:The onset of sensory and motor blockade was significantly faster in the clonidine groups, particularly in Group C (P < 0.001). Time to attain maximum sensory and motor blockade was also significantly reduced in Group C. Duration of motor block and postoperative analgesia increased with increasing clonidine dose. Mean duration of analgesia washighest in Group C (360.76 ± 55.21 min), followed by Group B (300.68 ± 43.20 min) and Group A (255.80 ± 33.61 min) (P < 0.001). Postoperative VAS scores at 4 hours were significantly lower in Group C. Although intraoperative heart rate and blood pressure were lower in the clonidine groups, the changes remained clinically manageable. Conclusion:Intrathecal clonidine with hyperbaric levobupivacaine improved block characteristics and prolonged postoperative analgesia in a dosedependent manner. Clonidine 30 μg provided longest analgesia but caused greater, clinically manageable hemodynamic effects.

Comparison of Different Dosage of Intrathecal Morphine as an Adjuvant to Hyperbaric Ropivacaine in Lower Limb Orthopaedic Surgeries: A Double-blinded Randomised Controlled Study

Introduction: Spinal anaesthesia with hyperbaric bupivacaine is the cornerstone of lower limb surgeries. With introduction of commercially prepared hyperbaric ropivacaine, its use has increased considerably as an effective alternate. Aim: To evaluate the analgesic and blockade characteristics of different dosages of m...

COMPARISON OF TWO DIFFERENT DOSES OF INTRATHECAL DEXMEDETOMIDINE (5 µG VERSUS 10 µG) AS AN ADJUVANT TO HYPERBARIC BUPIVACAINE FOR SPINAL ANAESTHESIA IN LOWER LIMB ORTHOPAEDIC SURGERIES: A PROSPECTIVE RANDOMIZED DOUBLE-BLIND STUDY

Background: Intrathecal dexmedetomidine (Intrathecal Dexmedetomidine) is increasingly used as an adjuvant to hyperbaric bupivacaine (Hyperbaric Bupivacaine) to increase the extent and spinal anaesthesia’s quality. However, the optimal intrathecal dose for elective orthopaedic procedures on the lower limbs remains uncer...

EFFECT OF INTRATHECAL DEXMEDETOMIDINE AS AN ADJUVANT TO BUPIVACAINE IN SPINAL ANESTHESIA: A PROSPECTIVE COMPARATIVE STUDY

Background: Spinal anesthesia using bupivacaine is widely employed for lower abdominal and lower limb surgeries. However, its limited duration of action often necessitates the use of adjuvants. Dexmedetomidine, a highly selective α2-adrenergic agonist, has shown promising results in prolonging sensory and motor blockad...

Dexmedetomidine As An Adjuvant To Levobupivacaine In Ultrasound-Guided Supraclavicular Brachial Plexus Block For Upper-Limb Orthopaedic Surgery: A Prospective Randomized Double-Blind Study

Background Supraclavicular brachial plexus block (SCBPB) is a common regional anaesthesia technique used for upper limb surgeries. This block provides safe and effective anaesthesia with excellent post-operative analgesia.  Local anesthetics alone sometimes provide suboptimal analgesia and delayed onset time, promptin...

Comparison of the Effect of Intrathecal Morphine and Intrathecal Fentanyl as Adjuvants to 0.5% Levobupivacaine in Total Knee Replacement Surgery

Background: Intrathecal opioids are commonly used as adjuvants to local anaesthetics to prolong spinal analgesia. Morphine (hydrophilic) provides prolonged analgesia; fentanyl (lipophilic) provides rapid onset but shorter duration. This study compared intrathecal morphine and fentanyl as adjuvants to 0.5% hyperbaric le...

PROSPECTIVE OBSERVATIONAL STUDY TO ASSESS TWO DIFFERENT DOSES OF BUPRENORPHINE (30 µG AND 75 µG) GIVEN INTRATHECALLY ALONG WITH HYPERBARIC BUPIVACAINE AS ANALGESIC IN PATIENTS UNDERGOING LOWER LIMB ORTHOPEDIC SURGERIES

Background: Effective postoperative pain management is essential following lower limb orthopedic surgeries, as inadequate analgesia may delay mobilization, prolong hospital stay, and adversely affect patient recovery. Intrathecal buprenorphine is a commonly used opioid adjuvant that enhances the duration and quality of...
