TL;DR

Available evidence suggests that this approach is associated with faster postoperative recovery and a reduced risk of postoperative dislocation, supporting the MIS-AL approach as a reliable and muscle-sparing option for primary THA.

Abstract

The anterolateral approach has been widely adopted in total hip arthroplasty (THA), with numerous modifications described since its original introduction. Among these, the minimally invasive anterolateral (MIS-AL) approach, also referred to as the anterior-based muscle-sparing approach, has been developed to minimize soft-tissue damage by preserving the integrity of the gluteus medius muscle. This narrative review describes the surgical technique of the MIS-AL approach for primary THA performed in the supine position and provides a summary of clinical outcomes. When performed in the supine position, the MIS-AL approach offers several advantages beyond its muscle-sparing nature. It does not require a specialized operating table, allows for a simplified operating room setup, facilitates the use of intraoperative fluoroscopy, and enables simultaneous bilateral THA without the need for re-draping. In addition, available evidence suggests that this approach is associated with faster postoperative recovery and a reduced risk of postoperative dislocation, supporting the MIS-AL approach as a reliable and muscle-sparing option for primary THA.

Direct anterior approach for femoral component revision: Surgical technique and pearls.

The direct anterior approach (DAA) has gained widespread acceptance for primary total hip arthroplasty because it utilizes an intermuscular and internervous interval, minimizes soft tissue disruption, and facilitates rapid postoperative recovery. Although its application in primary arthroplasty is well established, fem...

The Anatomic–Atraumatic–Anterior Reconstructive Technique (A-A-ART) for total hip arthroplasty: A step-by-step pictorial guide to a minimally invasive approach for hip osteoarthritis

Total hip arthroplasty remains one of the most successful orthopedic procedures for the treatment of hip osteoarthritis. Among contemporary surgical approaches, the direct anterior approach has gained increasing popularity due to its intermuscular and internervous plane, which aims to minimize soft-tissue trauma and po...

Anatomical Variations Encountered During the Posterior Approach to the Hip Joint

The posterior approach to the hip joint remains widely used for primary total hip arthroplasty because it provides extensive exposure while preserving the abductor mechanism. Safe execution, however, depends on recognizing anatomical variation within the gluteal region. This narrative review synthesizes variations enco...

Direct Anterior Approach to Hip Resurfacing Arthroplasty

Background: Hip resurfacing arthroplasty remains an appealing option for young and active patients1, but its use has declined because of technical challenges, risks of femoral neck notching, and adverse reactions associated with metal-on-metal bearings, raising concerns regarding long-term implant survival2. Accurate p...

Short-Term Clinicoradiological Outcomes of Primary Total Hip Arthroplasty With and Without Intraoperative Fluoroscopic Guidance: A Comparative Prospective Non-randomised Controlled (Quasi-experimental) Study

Abstract Introduction: The long-term success of total hip arthroplasty (THA) requires optimal acetabular component positioning. To improve implant placement, intra-operative fluoroscopy is increasingly used, yet the evidence remains limited in the Indian population and confined to the direct anterior approach. This stu...

Direct Anterior or Posterolateral Approach for Primary Total Hip Arthroplasty? A Systematic Review of Early Recovery, Long-Term Outcomes, and Surgical Complications

The optimal surgical approach for primary total hip arthroplasty (THA) remains debated. The direct anterior approach (DAA) has been associated with faster early recovery, whereas its potential advantages over posterior or posterolateral approaches (PA/PLA) in terms of longer-term outcomes and complications remain uncer...
