The Problem of Resection Margins in Soft Tissue Sarcomas of the Head and Neck in Children
Abstract
This article examines the interpretation and clinical significance of resection margins in soft tissue sarcomas of the head and neck in children, particularly in rhabdomyosarcoma, which is the most common type of tumor in this group in pediatric practice. It examines the current understanding of soft tissue sarcomas as a heterogeneous group of malignant neoplasms requiring a risk-adapted and multimodal approach, including surgical treatment, drug therapy, and radiation therapy. Particular attention is paid to the anatomical specifics of the head and neck region, where the high concentration of vital structures and the limited surgical space significantly complicate the achievement of radical resection without significant functional and cosmetic consequences. The review provides a detailed comparison of parameningeal and non-parameningeal tumor locations, highlighting their differing prognostic significance and variations in surgical treatment options. For parameningeal tumors, primary radical resection is often impossible or associated with an unacceptable risk of complications; therefore, in these cases, chemotherapy and radiation therapy form the basis of local control. In contrast, for non-parameningeal tumors, in some cases, achieving complete microscopic resection allows for a reduction in the extent of adjuvant therapy and even the omission of radiation therapy in certain patients. The article also discusses existing classifications of resection margins and notes the lack of a single, reproducible standard for their assessment, which complicates the interpretation of study results and the development of uniform surgical approaches. The authors emphasize that the status of the resection margin remains one of the key factors in local control: the risk of local recurrence increases with the transition from R0 to R1 and R2 resections. At the same time, the significance of this parameter should not be assessed in isolation but in conjunction with tumor location, histological type, extent of disease, patient age, and the feasibility of combined therapy. The prospects for organpreserving and minimally invasive techniques are discussed separately, as is the need for standardization of surgical and morphological assessment. In conclusion, it is concluded that further improvement in the treatment of children with head and neck rhabdomyosarcoma is possible if criteria for evaluating resection margins are standardized, an interdisciplinary approach is developed, and data from prospective studies are accumulated.
