TL;DR

Late-onset PA can present as isolated cardiomyopathy in adolescence, particularly in high-risk populations and early consideration of metabolic etiologies is essential in unexplained DCM, particularly in high-risk populations.

Abstract

Background Propionic acidemia (PA) is a rare inborn error of metabolism that typically presents in infancy; however, late-onset variants may manifest with atypical features, including cardiac involvement. Dilated cardiomyopathy (DCM) is an uncommon but serious complication. Case summary A 15-year-old with no prior medical history but a significant maternal family history of cardiomyopathy of unknown etiology from an Amish background presented with altered mental status, increased work of breathing, and progressive edema in the context of preceding viral illness. Evaluation revealed severe biventricular dysfunction, chamber dilation, and findings consistent with chronic DCM. Despite supportive management, her course was complicated by ventricular arrhythmias and clinical deterioration. Metabolic workup demonstrated elevated propionylcarnitine and increased urinary 2-methylcitric acid, confirming late-onset variant PA. She was treated with intravenous carnitine and a specialized metabolic formula; however, cardiac function remained severely impaired. In accordance with family wishes, she was discharged home on hospice care. Conclusion Late-onset PA can present as isolated cardiomyopathy in adolescence. Early consideration of metabolic etiologies is essential in unexplained DCM, particularly in high-risk populations.

Progressive Dilated Cardiomyopathy in a 24-Year-Old Man: A Rare Manifestation of Propionic Acidemia.

BACKGROUND Adult onset propionic acidemia (PA) is exceedingly rare, particularly when manifesting as dilated cardiomyopathy. CASE SUMMARY A previously healthy 24-year-old man presented with cardiac dilatation and acute progressive heart failure. His condition was refractory to guideline-directed medical therapy, dete...

Supraventricular Tachycardia as an Unusual Presentation of Infantile Onset Pompe Disease: A Case Report

Pompe disease is a rare autosomal recessive lysosomal storage disorder with an incidence of approximately one in 40,000 live births is caused by deficiency of acid α-glucosidase, leading to progressive glycogen accumulation in cardiac and skeletal muscle. Infantile Onset Pompe Disease (IOPD) represents the most severe...

Case Report: Duchenne muscular dystrophy–associated cardiomyopathy presenting as adolescent nonischemic heart failure—from red-flag clues to genetic confirmation and multisystem management

Background Duchenne muscular dystrophy (DMD) can involve the myocardium and progress to cardiomyopathy and heart failure; however, diagnosis is frequently delayed because initial clinical manifestations are often atypical. In adolescent males presenting with nonischemic cardiomyopathy of unclear etiology, early integra...

Endocardial thickening in dilated cardiomyopathy associated with FHL2 variant: a case report

Background FHL2 is critical for cardiac development, and its variants are associated with early-onset severe dilated cardiomyopathy (DCM), though such reports remain rare. Case presentation The patient was a 30-day-old male infant admitted with a 3-day history of cough, a 2-day history of poor feeding, and lethargy las...

Severe Hypercalcemia-Induced Electrocardiographic Abnormalities as the Presenting Manifestation of Multiple Myeloma: A Case Report

Background: Severe hypercalcemia may cause neuropsychiatric manifestations, acute kidney injury, and characteristic electrocardiographic (ECG) abnormalities. In older adults, these findings may mimic acute coronary syndromes or primary rhythm disorders, resulting in diagnostic delay. Case presentation: An 82-year-old w...

Case Report: Brugada syndrome uncovered by delivery in a 32-year-old patient

We report a 32-year-old primigravida with no prior symptoms, normal baseline ECG, and no family history of sudden death who developed ventricular fibrillation (VF) shortly after uncomplicated vaginal delivery. She received epidural ropivacaine and sufentanil during labor. Post-resuscitation ECG demonstrated coved ST-se...
