Choosing depression treatment in type 2 diabetes: integrating psychiatric efficacy, metabolic effects, and cardiovascular risk. A narrative review

Abstract

Depression in people with type 2 diabetes (T2D) complicates daily self-management and treatment selection. Psychiatric benefit must be considered alongside weight, glycaemia, cardiovascular tolerability and interactions. This narrative review draws on purposively selected literature and distinguishes direct T2D evidence from mixed-diabetes studies, observational associations and indirect evidence. Study selection and extraction were not consistently duplicated, and no formal de novo risk-of-bias or GRADE assessment was undertaken. A revision-stage qualitative appraisal makes study limitations and population differences explicit. The main T2D pharmacotherapy network comprised 12 short-term trials with 792 participants; its sparse comparisons do not establish a reliable prescribing hierarchy. Individual-agent data suggest clinically relevant differences, but favourable mean HbA1c changes do not establish freedom from hypoglycaemia or hyperglycaemia. Psychological, exercise, digital and collaborative-care interventions also differ in diagnostic eligibility, support intensity and metabolic findings. Cardiovascular tolerability, intermediate safety markers and prevention of clinical events require separate interpretation. Cardiac studies cannot establish T2D-specific cardioprotection. We propose an unvalidated conceptual checklist in which psychiatric urgency precedes cardiometabolic constraints, prior response and patient preferences. Monitoring examples are extrapolated from guidance and pharmacological safety information, not tested T2D-specific schedules. The checklist is intended to structure discussion, not to function as a guideline, validated decision aid or prediction rule.
