Abstract

Respiratory exposures remain among the leading preventable contributors to lung cancer and other chronic respiratory diseases. Yet, existing surveillance resources are fragmented, provide static annual estimates, and lack integrated real-time geospatial monitoring across multiple exposure types. We developed the Respiratory Exposure Observatory (REO) - dynamic nationwide surveillance platform integrates data from large-scale research cohorts and publicly available resources, including the CDC, U.S. Census Bureau, American Community Survey (ACS), EPA, and national open API datasets. REO combines information on combustible tobacco use, passive and thirdhand smoke exposure, electronic nicotine delivery systems (vaping), hookah and heated tobacco products, cannabis smoking, air pollution, occupational inhalation exposures, socioeconomic factors, healthcare accessibility, and respiratory disease burden. Using spatiotemporal statistical modeling, Bayesian hierarchical modeling, geospatial analytics, and explainable artificial intelligence (XAI), REO enables interactive, real-time visualization of respiratory exposure patterns, disparity-associated hotspots, and communities at elevated risk. This scalable platform supports lung cancer prevention, environmental health research, respiratory health surveillance, precision public health, and cancer health disparities research through continuously updated geospatial intelligence.
