The combined approach delivers structural repair, mechanical reinforcement and flow modulation in one stage and avoids prosthetic interposition, separate arterial dissection and prolonged catheter dependence.

Abstract

True aneurysms of a haemodialysis arteriovenous fistula (AVF) and a co-existent high-flow state pose a particular surgical problem, because the structural and the haemodynamic abnormality must be corrected at the same operation if a precious native access is to be preserved. We report a 51-year-old woman maintained on thrice-weekly haemodialysis through an 18-month-old left brachiocephalic AVF, who developed two cephalic-vein true aneurysms (2.48 and 2.14 cm) in the outflow segment with an access flow of 6.7 L/min,(peak-systolic-velocity-based duplex estimate).concentric left-ventricular hypertrophy and grade 3 diastolic dysfunction Through a single incision we performed partial-wall aneurysmorrhaphy of both sacs, deliberate downstream venous plication for flow reduction, and circumferential external polytetrafluoroethylene (PTFE) reinforcement of the entire repaired and plicated segment, with intraoperative duplex titration of the haemodynamic endpoint. Access flow fell from 6.7 to 1.4 L/min (≈79% reduction) with preserved patency. The combined approach delivers structural repair, mechanical reinforcement and flow modulation in one stage and avoids prosthetic interposition, separate arterial dissection and prolonged catheter dependence.

Intravascular "fenestration" and "triangular anchoring" techniques: A case of uncaptured central venous retrograde stent placement and a literature review.

A 61-year-old woman with stenosis at the left brachiocephalic vein-superior vena cava junction and symptomatic retrograde migration of a bare-metal stent impacted in the axillary vein is presented and an innovative endovascular rescue was performed: intrastent fenestration with triangular-complex anchoring.

URGENT SURGICAL REPAIR OF AN EXPANDING ARTERIOVENOUS FISTULA PSEUDOANEURYSM IN A PATIENT WITH MODERATE DILATED CARDIOMYOPATHY: A CASE REPORT

Background: Arteriovenous fistula (AVF) pseudoaneurysms are critical complications demanding immediate evaluation. It is a medical emergency requiring prompt management, as conservative measures are inadequate for rapidly expanding masses due to the high risk of fatal haemorrhage. Case description: A 54-year-old female...

Stepwise Incisionless Percutaneous Flow-Control Treatment of a High-Flow Scalp Arteriovenous Fistula with a Large Superficial Venous Pouch: A Case Report

Objective Scalp arteriovenous fistulas (AVFs) with multiple tortuous superficial feeders and a large venous pouch can require meticulous flow control during treatment. Open surgical excision may be associated with blood loss, wound-healing problems, and cosmetic concerns, whereas percutaneous liquid embolization of a h...

Radial Artery Aneurysm Requiring Reconstruction Following Previous Ulnar Artery Ligation: A Case Report Motaz Joma Hamad

Radial artery aneurysms are uncommon, and management is particularly challe, nging when alternative arterial perfusion of the hand is inadequate. A 70-year-old woman presented with a three-month history of a slowly enlarging pulsatile swelling in the right anatomical snuffbox. Ten years earlier, she had sustained a lace...

A ruptured internal carotid artery blood blister-like aneurysm with rebleeding after single-layer neck-bridging stenting and complete obliteration after double-layer stenting: A case report

Blood blister-like aneurysms (BBAs) are difficult to treat surgically, and an established treatment strategy is yet to be determined. Treatment options are limited in cases where direct surgery is difficult, and endosaccular embolization is considered infeasible. A 31-year-old man presented with subarachnoid...

Successful Embolization of a Persistent Left Superior Vena Cava Using the Preloading Coil-in-Plug Technique after Glenn Surgery: A Case Report

: Background: Systemic venous collaterals after Glenn surgery may cause persistent cyanosis. The coil-in-plug (CIP) technique can provide strong occlusion within a short landing zone. Additionally, the preloading CIP (pCIP) technique, which involves preloading a microcatheter through the Amplatzer vascular plug (AVP) b...
