This case of a 14-year-old female with a Todani type IVa choledochal cyst complicated by numerous stones highlights the limitations of laparoscopic surgery for congenital biliary dilation with advanced hepatolithiasis.

Abstract

Although postoperative intrahepatic stones are known to occur after surgery for congenital biliary dilation, preoperative diffuse hepatolithiasis is uncommon. We present a challenging case of a 14-year-old female with a Todani type IVa choledochal cyst complicated by numerous stones. She presented with abdominal pain, obstructive jaundice, and pancreatitis. To stabilize her condition, she was initially treated with endoscopic biliary and pancreatic stenting. Subsequently, laparoscopic radical surgery was performed. Although many stones were removed intraoperatively, complete clearance remained elusive despite repeated lavage and cholangioscopy. Following surgery, the patient recovered from a pelvic abscess and was discharged on postoperative Day 17. MRCP at 3 months showed residual stones, although the patient remained asymptomatic. This case highlights the limitations of laparoscopic surgery for congenital biliary dilation with advanced hepatolithiasis. The indications for open surgery should be carefully evaluated, and treatment strategies should anticipate the possibility of postoperative residual stones.

Cholangitis, Choledocholithiasis and a Cholecystocolonic Fistula: A Case Report on a Rare Triple Surgical Challenge

Cholecystocolonic fistula (CCF) is an uncommon complication of gallstone disease and is usually diagnosed intraoperatively. Concomitant choledocholithiasis and acute cholangitis create competing priorities: urgent biliary drainage and definitive treatment of the fistula. A 78‐year‐old woman presented with 3 days of upp...

Postoperative Cholestasis After Uncomplicated Laparoscopic Cholecystectomy: A Case Report

Introduction In the absence of complications, transient jaundice has been observed in postoperative patients after abdominal surgery. This jaundice has been previously attributed to intrahepatic cholestasis due to oxidative stress from surgery and can be marked within the first postoperative weeks. However, this change...

Choledochal Cysts: Contemporary Diagnosis, Surgical Management, and Long-Term Surveillance

Choledochal cysts, increasingly referred to as congenital biliary dilatation, are uncommon abnormalities of the biliary tree associated with biliary stasis, pancreatobiliary reflux, recurrent inflammation, pancreatitis, cholangitis, stone formation, progressive hepatobiliary injury, and an increased lifetime risk of bi...

Suspected Groove Pancreatitis Complicated by Anastomotic Stricture and Recurrent Intrahepatic Lithiasis: A Case Report

Groove pancreatitis is a rare form of chronic pancreatitis involving the pancreatoduodenal groove and may mimic pancreatic or periampullary malignancy, creating substantial diagnostic uncertainty. We report a 67-year-old man with a history of alcohol use, a 10-pack-year smoking history, chronic pancreatitis, and suspec...

Symptomatic intrahepatic subcapsular hematoma-derived liver cyst following laparoscopic cholecystectomy: a case report

Background Intrahepatic subcapsular hematoma (ISH) is a rare complication following laparoscopic cholecystectomy (LC). Although most reported cases occur in the early postoperative period and resolve with conservative treatment, the management of chronic or refractory ISH remains unclear. We report a delayed, symptomat...

Bile Leak 30 Years Post-cholecystectomy: A Rare Case

Bile leaks are a well-recognised complication of cholecystectomy, typically presenting within the first postoperative week. Extremely delayed presentations are exceptionally rare, with only a handful of cases reported years after the index procedure. We report the case of a 60-year-old woman who presented with a four-d...
