According to medium-term follow-up results, the medial femoral condyle flap is a safe and reliable method for the reconstruction of TMJ defects.

Abstract

Defects of the temporomandibular joint (TMJ) severely compromise mandibular function and facial contour, rendering its reconstruction one of the most challenges in maxillofacial surgery. This study introduced a novel technique for TMJ reconstruction employing vascularized medial femoral condyle (MFC) osteochondral flap and aimed to evaluate its medium-term functional outcomes and stability. This observational study included 15 patients who received vascularized MFC osteochondral flap reconstruction for TMJ ankylosis or neoplasms. The follow-up ranged from 12 to 69 months (median:24 months). The average of postoperative maximal mouth opening (MMO) was significantly increased compared with the pre-operation in patients with TMJ ankylosis (p = 0.0039). The postoperative MMO in patients with TMJ neoplasms was comparable with preoperative (p = 0.2500). The average ramus height remained stable in both TMJ ankylosis (p = 0.1875) and neoplasms (p = 0.8125) without significant difference. All donor sites healed favorably, with Lower Extremity Functional Scale (LEFS) scores ranging from 70 to 80 (maximum 80). According to medium-term follow-up results, the medial femoral condyle flap is a safe and reliable method for the reconstruction of TMJ defects.

Evaluation of Postoperative Facial Asymmetry Index Following Mandibular Reconstruction Using Free Fibular Flap With Condylar Involvement.

BACKGROUND Loss of the temporomandibular joint (TMJ) following mandibulectomy disrupts maxillomandibular continuity and occlusion, potentially resulting in facial asymmetry with functional, structural, and aesthetic consequences. The free fibular flap remains the gold standard for mandibular reconstruction. However, th...

Condyle-preserving lateral arthroplasty for fibro-osseous temporomandibular joint ankylosis using customized cutting guides: a case report

Temporomandibular joint ankylosis causes progressive restriction of mandibular movement and is primarily managed surgically, although the need for routine condylar resection remains controversial. Virtual surgical planning and patient-specific cutting guides may facilitate condyle-preserving resection, but their...

Temporomandibular Joint Defects: Etiology and Reconstruction Strategy: A Narrative Review

Temporomandibular joint (TMJ) defects primarily arise from congenital malformations, bony ankylosis, and radical tumor resection. As a pivotal structure for regulating maxillofacial development and maintaining stomatognathic homeostasis, the anatomical restoration and functional reconstruction of the TMJ are essential...

Correlation between preoperative mandibular restriction and functional outcomes after arthroscopic discopexy for temporomandibular joint internal derangement

Background/purpose Arthroscopic discopexy is an established treatment for temporomandibular joint (TMJ) internal derangement, yet its functional benefit in patients with restricted mouth opening remains unclear. This study evaluate whether arthroscopic discopexy improves mouth opening and pain in patients with restrict...

Mouth opening after fibula free flap reconstruction: Impact of radiation and implications for prosthodontic rehabilitation.

STATEMENT OF PROBLEM Mandibular mobility is critical for prosthodontic rehabilitation following maxillofacial reconstruction, especially in a procedure like Jaw-in-a-Day (JIAD) that needs accurate pre-operative planning. The fibula free flap (FFF) reconstruction enables the restoration of appearance and function after...

Custom-Made Total Temporomandibular Joint Replacement: A Long-Term Observational Case Series of Four Patients

Background/Objectives: Advanced acquired or congenital temporomandibular joint (TMJ) pathology may severely impair oral function and quality of life, often requiring total joint replacement when conservative and reconstructive treatments fail. Custom-made TMJ prostheses (CMTMJP) have been developed to improve anatomica...
