TL;DR

Flow cytometry can provide useful information beyond CAR-T-cell enumeration and may support longitudinal immune monitoring after CAR-T-cell therapy, and may support longitudinal immune monitoring after tisagenlecleucel infusion.

Abstract

Simple Summary Chimeric antigen receptor (CAR) T-cell therapy is an effective treatment for several difficult-to-treat B-cell cancers, but the immune changes that occur after treatment are not fully understood. Most monitoring focuses primarily on circulating CAR-T cell counts. In this study, we used multiparameter flow cytometry to monitor both CAR-T cells and endogenous T cells during the first month after tisagenlecleucel infusion. CAR-T cells showed changes in expansion, differentiation, and expression of immune-related markers over time, while their overall differentiation pattern was broadly similar to that of endogenous T cells. Patients who developed neurotoxicity also showed higher absolute CAR-T cell counts in an exploratory analysis. These findings suggest that flow cytometry can provide useful information beyond CAR-T-cell enumeration and may support longitudinal immune monitoring after CAR-T-cell therapy.

Population cellular kinetics and exposure-CSR analysis of CAR-T cells in Chinese patients with relapsed/refractory B-ALL

Background CD19-targeting chimeric antigen receptor (CAR)-T cells have demonstrated great efficacy in treating relapsed and refractory B-cell acute lymphoblastic leukemia (B-ALL). Despite the high objective response rate of CAR-T cell therapy, high-risk adverse event of cytokine-release syndrome occur unexpectedly. Obj...

Distinct cellular dynamics following CD19 and CD7 CAR-T therapies revealed by high-dimensional CyTOF analysis and impact on clinical outcomes

Compared with CD19 CAR-T therapy, CD7 CAR-T therapy was characterized by reduced CD8+ CAR-T persistence, early exhaustion, and delayed immune reconstitution, correlating with increased infections and higher non-relapse mortality.

Blood Cell Biophysical Trajectories Reveal Response to CAR-T Therapy in Diffuse Large B-Cell Lymphoma.

Although chimeric antigen receptor T-cell (CAR-T) therapy has substantially improved outcomes in diffuse large B-cell lymphoma (DLBCL), disease relapse remains a major clinical challenge. Because treatment-associated immune remodeling evolves dynamically over time, complementary biomarkers capable of longitudinally mon...

Model-based integration of chimeric antigen receptor T-cell kinetics and tumor features to predict therapeutic outcomes in patients with B-cell malignancies

Introduction Chimeric antigen receptor (CAR) T-cell therapy is a standard treatment for several hematological malignancies, yet patient-specific responses and toxicities remain hard to predict. Although multiple covariates and risk scores exist, their prognostic accuracy remains limited. Methods We prospectively measur...

Rapid Manufactured CAR-T Cells enriched for CD45RA-negative T cells Exhibit Superior Persistence Compared to Conventional CAR-T Therapy.

CD19 CAR-T has achieved high remission rates in relapsed/refractory B-cell acute lymphoblastic leukemia (B-ALL). However, rapid generation with durable in vivo persistence remains a major challenge. CD45RA-negative T cells exhibit sustained antileukemic activity and preserved immune memory. Here, we developed a rapid p...
