TL;DR

Only double-positive tumors (PCR+/p16+) show the prognostic benefit of HPV-related oropharyngeal carcinomas, and the combination of PCR and p16 improves prognostic accuracy and allows for the adequate identification of patients who are candidates for specific therapeutic strategies.

Abstract

INTRODUCTION The characterization of human papillomavirus (HPV) infection in oropharyngeal carcinomas is prognostically relevant, but there may be discordance between p16 expression and molecular detection of viral DNA.

Materials And Methods

Retrospective study of 620 patients with squamous cell carcinoma of the oropharynx treated between 1991 and 2024 with joint determination of HPV by PCR and p16. Epidemiological, clinical, and therapeutic characteristics were analyzed according to HPV status, as well as specific, overall, and recurrence-free survival.

Results

Overall, 74.0% of tumors were PCR-/p16-, 19.4% were PCR+/p16+, and 6.6% were discordant (3.9% PCR+/p16-; 2.7% PCR-/p16+). Five-year specific survival was significantly higher in the PCR+/p16+ group (P = 0.0001). No differences in survival were observed between the discordant and double-negative groups (P = 0.311). In the multivariable analysis, considering PCR+/p16+ patients as the reference category, the risk of death from tumor was 4.97 times higher in PCR+/p16-, 5.42 times higher in PCR-/p16+, and 3.63 times higher in PCR-/p16-. Similar findings were obtained in the cohort treated with radical intent. The prognostic differences remained for deaths from oropharyngeal tumor, second neoplasm, or non-oncological causes.

Conclusions

Only double-positive tumors (PCR+/p16+) show the prognostic benefit of HPV-related oropharyngeal carcinomas. Discordant tumors had a similar outcome to double-negative tumors. The combination of PCR and p16 improves prognostic accuracy and allows for the adequate identification of patients who are candidates for specific therapeutic strategies.

HPV Detection by RT-PCR in Laryngeal Squamous Cell Carcinoma: Correlation with p16 Expression, Histopathology, and Prognosis.

OBJECTIVE The aim of the study was to determine HPV incidence among laryngeal squamous cell carcinomas (LSCC) using HPV E6/E7 mRNA RT-PCR and to compare diagnostic capabilities of p16 immunohistochemistry against it. With the data gathered, the relationship between HPV status, pathological and clinical parameters will...

Prognostic implications of HPV and TP53 status in early-stage vulvar squamous cell carcinoma.

OBJECTIVE To evaluate the prognostic significance of HPV and p53 status in patients with FIGO 2021 stage I vulvar squamous cell carcinoma (vSCC) treated with surgery alone. METHODS We retrospectively identified patients with stage I vSCC treated surgically between 2000 and 2021 with available tissue for p16 immunohis...

p16 Overexpression and Human Papillomavirus Positivity in More Than 1000 Penile Cancers

Penile cancer is a severe disease, and a large proportion is related to high‐risk (hr) human papillomavirus (HPV). P16 is used as an immunohistochemical marker of HPV in clinical diagnostics. Population‐based data on the prevalence of p16 in penile cancer is scarce. We investigated the prevalence of p16 positivity, and...

HPV16 transcriptional patterns in oropharyngeal squamous cell carcinoma with different clinical outcomes

Overall, this exploratory study identifies HPV16 transcriptional differences between OPSCC tumors from patients with different clinical outcomes and requires validation in larger, independent cohorts before prognostic or clinical relevance can be inferred.

Clinicopathologic features and molecular characterization of oropharyngeal carcinomas harboring low-risk human papillomavirus.

While the significance of LR-HPV in these neoplasms remains uncertain, the findings expand the spectrum of HPV-associated lesions encountered in the oropharynx and provide a foundation for future mechanistic studies.

Differential prognostic value of PD-L1 expression in oropharyngeal squamous cell carcinoma based on HPV status.

OBJECTIVE To investigate the correlation between PD-L1 expression and HPV status in oropharyngeal squamous cell carcinoma (OPSCC) and evaluate their prognostic implications. METHODS We retrospectively analyzed 131 OPSCC patients treated at the Affiliated Hospital of Qingdao University. PD-L1 and p16 expression were e...
