A patient with a remote history of a left cerebellar lesion resection with unknown pathology presented with sensation changes extending caudally from the torso and underwent laminectomy and tumor resection of the lesion, resulting in recurrence at 1.5 months requiring re-resection for palliative decompression.
Abstract
BACKGROUND Primary CNS sarcoma, DICER1-mutant is almost exclusively found in the supratentorial compartment. Isolated spinal lesions are exceedingly rare. OBSERVATIONS The authors present a case of thoracic intradural extramedullary primary CNS sarcoma, DICER1-mutant. An adult male with a remote history of a left cerebellar lesion resection with unknown pathology presented with sensation changes extending caudally from the torso. He subsequently underwent laminectomy and tumor resection of the lesion. The patient experienced recurrence at 1.5 months requiring re-resection for palliative decompression. LESSONS Molecular characterization is essential for accurate diagnosis of this rare entity given its substantial overlap with histological mimics. Following resection and tissue identification of the tumor, patients should be monitored closely for local recurrence. While no standardized treatment exists, adjuvant chemoradiation therapy may improve time to progression and overall survival in an otherwise aggressive disease. https://thejns.org/doi/10.3171/CASE26660
Abstract Myxofibrosarcoma (MFS) is a fibroblastic sarcoma that typically arises in the extremities of elderly patients; primary retroperitoneal involvement is exceptionally rare and often mimics myxoid liposarcoma. We report a 53-year-old man with a three-month history of progressive abdominal distension caused by a gi...

Primary extrarenal nephroblastoma, or extrarenal Wilms tumor, is an exceptionally rare embryonal tumor outside the kidney. Spinal/lumbosacral involvement is particularly uncommon and may mimic congenital dysraphic lesions. We report a 10-month-old female infant followed since birth for presumed sacral lipoma and operat...

BACKGROUND Spindle cell oncocytoma (SCO) is a rare, nonadenomatous sellar tumor that often mimics pituitary adenomas both radiographically and clinically, making diagnosis and treatment difficult. OBSERVATIONS A 67-year-old male presented with fatigue and weakness, with MRI revealing an enhancing sellar mass with supra...
