TL;DR
It is concluded that the standardized use of LI-RADS within a sequential ultrasound–MRI approach increases the reliability of noninvasive diagnosis of HCC and reduces the number of unnecessary biopsies.
Abstract
Liver cirrhosis is the main background for the development of hepatocellular carcinoma (HCC), and the detection of a focal lesion in a cirrhotic liver always requires its differentiation from regenerative and dysplastic nodules, benign pseudolesions and non-hepatocellular malignancies. This review analyzes current data on the role of ultrasound (B-mode, Doppler imaging, elastography, contrast-enhanced ultrasound) and magnetic resonance imaging (MRI) with diffusion-weighted imaging, extracellular and hepatobiliary contrast agents in this patient group. The imaging correlates of multistep hepatocarcinogenesis are described. The structure of the Liver Imaging Reporting and Data System (LI-RADS) is presented, including its ultrasound surveillance, contrast-enhanced ultrasound and CT/MRI algorithms, diagnostic categories and management recommendations. The main pitfalls of differential diagnosis are discussed: arterioportal shunts, confluent fibrosis, siderotic nodules and intrahepatic cholangiocarcinoma. It is concluded that the standardized use of LI-RADS within a sequential ultrasound–MRI approach increases the reliability of noninvasive diagnosis of HCC and reduces the number of unnecessary biopsies.
The liver is one of the most frequent sites of hematogenous metastasis, and metastatic lesions considerably outnumber primary malignant liver tumors. Accurate detection of liver metastases and their differentiation from benign focal lesions determine the stage of the disease and the choice between surgical, systemic an...
INTRODUCTION
Accurate classification of liver tumors requires contrast-enhanced MRI. However, contrast agents are contraindicated in patients with renal impairment or allergies. Concerns regarding the environmental impact of gadolinium-based agents have increased. Thus, classification using non-contrast MRI is desirabl...

LI-RADS AFs enhance HCC diagnostic accuracy when complementing major features, with the most robust evidence supporting MRI-based AFs, while AFs based on CT and CEUS require further validation.

The incidental discovery of focal liver lesions (FLLs) has reached unprecedented levels due to the widespread use of high-resolution imaging, yet unenhanced ultrasound often lacks the specificity required for definitive characterization. This review identifies Contrast-Enhanced Ultrasound (CEUS) as a transformative dia...

OBJECTIVES
Accurate differential diagnosis of benign and malignant superficial lymph nodes (LNs) is critical for clinical management. The objectives of this study were to explore the feasibility and clinical value of super-resolution ultrasound (SRUS) in the differential diagnosis of superficial lymphadenopathy and to...

Early detection of cirrhosis is essential for guiding management and prognostic surveillance for hepatocellular carcinoma. Although elastography-based techniques are increasingly used, abdominal computed tomography (CT) remains the most routinely performed imaging modality in chronic liver disease surveillance....
