TL;DR

This is the first published case describing cefepime PK in an adolescent on iHD and demonstrates the utility of model-informed precision dosing in this population.

Abstract

There are no studies evaluating cefepime pharmacokinetics (PK) in pediatric patients receiving intermittent hemodialysis (iHD). Cefepime dosing in this population is particularly challenging owing to several variables that can affect clearance, including dialysis prescription, residual kidney function, fluid overload, and systemic inflammation. We describe cefepime PK parameters and concentration time profiles in an adolescent male undergoing iHD for whom a cefepime PK consult was requested. Using model-informed precision dosing, we were able to ensure the patient's current dosing of cefepime at 1000 mg daily maintained estimated free cefepime concentrations above 4 times the minimum inhibitory concentration (MIC) for MICs up to ∼4 mg/L while avoiding excessively high trough concentrations. Despite suspicion for increasing residual kidney function, due to increasing urine output, the patient's PK parameters were very similar 4 months later, when an additional consult was requested. This is the first published case describing cefepime PK in an adolescent on iHD and demonstrates the utility of model-informed precision dosing in this population.

Dose Optimization of Cefazolin for Pediatric Patients with Renal Impairment Using Physiologically Based Pharmacokinetic Modeling (PBPK) in GastroPlus

Background/Objectives: Cefazolin is a first-generation cephalosporin antibiotic widely used for the treatment and prophylaxis of bacterial infections in both adult and pediatric populations. It is predominantly eliminated unchanged by the kidneys. Impaired renal function can reduce its clearance and increase systemic e...

Development and validation of a cefepime population pharmacokinetic model to guide treatment for patients with hospital-acquired pneumonia requiring renal replacement

ABSTRACT Cefepime is used in critically ill pneumonia patients, but dosing in renal replacement therapy (RRT) is complicated by altered renal function, extracorporeal removal, and variability in pulmonary epithelial lining fluid (ELF) exposure. We sought to develop an integrated model linking systemic, extracorporeal,...

From historical dosing to precision dosing: Modernising tobramycin monitoring in people with cystic fibrosis

Historical therapeutic drug monitoring (TDM) strategies for intravenous tobramycin in people with cystic fibrosis (pwCF) were developed in the pre-CFTR modulator era and relied predominantly on clearance-based monitoring to minimise toxicity. Contemporary pwCF demonstrate greater variability in body composition and cli...

Vancomycin population pharmacokinetics and optimized dosing in critically ill infants and children.

BACKGROUND Vancomycin dosing remains challenging due to narrow therapeutic window and pharmacokinetic (PK) variability in critically ill children. This study aims to characterise pediactric vancomycin PK, identify significant covariates, and optimise dosing strategies. METHODS Vancomycin plasma concentrations from pa...

Vancomycin Population Pharmacokinetics and Exposure Variability in Children with Multiple Organ Dysfunction Syndrome.

In children with multiple organ dysfunction syndrome (MODS), vancomycin dosing is frequently guided by population-based approaches followed by therapeutic drug monitoring that requires considerable blood volumes. We sought to develop a population pharmacokinetic (PK) model and describe vancomycin exposures using blood-...

Cefepime exposure in prolonged or intermittent infusion in critically ill children.

AIMS Cefepime is commonly used in critically ill children for whom there is a high between-subject variability. To reach the therapeutic pharmacokinetic (PK) target in critically ill patients, prolonged infusion of beta-lactams seems to be the most efficient. The aim of this study was to compare cefepime exposure betwe...
