Current therapeutic approaches and emerging strategies in the management of DCIS and premalignant breast lesions are summarized, with a particular focus on risk stratification, treatment de-escalation, and future directions toward personalized care.

Abstract

Ductal carcinoma in situ (DCIS) and premalignant breast lesions represent a heterogeneous spectrum of non-invasive breast neoplasia with variable potential for progression to invasive breast cancer. The widespread use of screening mammography has led to a marked increase in DCIS detection, raising concerns about overtreatment and highlighting the need for risk-adapted management strategies. Current standard treatment for DCIS includes breast-conserving surgery or mastectomy, often followed by adjuvant radiotherapy to reduce local recurrence when breast conservative surgery (BCS) is performed. Endocrine therapy with tamoxifen or aromatase inhibitors is recommended for selected patients with hormone receptor-positive disease to lower the risk of ipsilateral and contralateral events. However, despite improved local control, these interventions have not consistently demonstrated a survival benefit, prompting reassessment of treatment intensity. Emerging strategies aim to individualize therapy by integrating clinicopathologic factors with molecular and genomic profiling to better stratify recurrence and progression risk. Ongoing clinical trials are evaluating active surveillance for carefully selected low-risk DCIS, as well as de-escalation approaches that may safely omit radiotherapy or surgery. Management of premalignant lesions such as atypical ductal hyperplasia increasingly emphasizes risk reduction, surveillance, and patient-centered decision-making. The aim of our review is to summarize current therapeutic approaches and emerging strategies in the management of DCIS and premalignant breast lesions, with a particular focus on risk stratification, treatment de-escalation, and future directions toward personalized care.

Adjuvant treatments for stage I breast cancers of one centimeter or less.

PURPOSE OF REVIEW The increasing detection of subcentimetric breast cancers (BC) through widespread implementation of mammographic screening has raised important questions regarding the optimal management of patients with stage I tumors measuring ≤1 cm in the adjuvant setting. Although these tumors exhibit an overall e...

Multicentric and multifocal lobular breast cancer: selection of radical surgical treatment

Background: invasive lobular breast carcinoma is characterized by a high frequency of multicentric and multifocal growth, which complicates the choice of the extent of surgery. The feasibility of breast-conserving surgery in this category of patients, particularly in early-stage disease, remains a matter of debate. Mos...

The challenge of evaluating the efficacy of neoadjuvant endocrine therapy in breast cancer and potential solutions: a literature review

Hormone receptor–positive (HR+) breast cancer is the most common subtype of the disease and is characterized by significant heterogeneity in its clinical course and response to therapy. Adjuvant endocrine therapy with aromatase inhibitors represents a highly effective alternative to chemotherapy in postmenopausal patie...

Genomic-Guided Adjuvant Therapy in Early-stage Endometrial Cancer: a Review

Current data support the incorporation of tumor genomic analysis into routine clinical decision-making as an important step toward precision-based care in endometrial cancer, and suggest that molecular profiling may better identify patients who would benefit from treatment intensification or de-escalation compared with...

Toward targeted therapeutics for lobular breast cancer

Despite growing recognition of invasive lobular carcinoma (ILC) as a biologically and clinically distinct subtype of breast cancer, ILC remains understudied. Most contemporary therapeutic trials continue to enroll patients predominantly with invasive ductal carcinoma/invasive carcinoma of no special type and rarely str...
