TL;DR

ERCP is a safe and effective initial treatment for post-transplant biliary strictures in children, and complications are generally minor and manageable, supporting its role as a minimally invasive therapeutic option in pediatric liver transplant centers.

Abstract

Biliary strictures are among the most frequent complications following pediatric liver transplantation and can compromise graft function if not promptly managed. Endoscopic retrograde cholangiopancreatography (ERCP) is increasingly recognized as a safe, minimally invasive first‐line therapy, though pediatric data remain limited.

Exploring the Role of Percutaneous Transhepatic Biliary Drainage in the Interim Management of Paediatric Choledochal Cysts with Cholangitis: A Pilot Study.

BACKGROUND Choledochal cysts are congenital anomalies of the bile ducts that can lead to significant complications, such as cholangitis, liver fibrosis, and biliary obstruction. This study aims to evaluate the effectiveness and safety of percutaneous transhepatic biliary drainage (PTBD) in managing choledochal cysts wi...

Does Stricture Location Predict ERCP Success in Non-Anastomotic Biliary Strictures After Living Donor Liver Transplantation? A Buis-Based Analysis

Non-anastomotic biliary strictures (NAS) represent one of the most challenging complications after living donor liver transplantation (LDLT) and are associated with heterogeneous outcomes following endoscopic retrograde cholangiopancreatography (ERCP). While ERCP is widely accepted as first-line therapy, the impact of...

Endoscopic Management of Biliary Strictures After Liver Transplantation: Current Evidence and Emerging Approaches.

Liver transplantation is a definitive treatment for end-stage liver disease and can improve quality of life and long-term prognosis. However, postoperative biliary complications remain an important limitation to transplant outcomes, and biliary stricture is among the most prevalent. Patients may present with jaundice,...

Should We Be Aiming For A "Stent-first Strategy" in the Management of Malignant Biliary Obstruction? A Retrospective Single-Center Analysis.

BackgroundPercutaneous transhepatic cholangiography (PTC) is essential when endoscopic retrograde cholangiopancreatography (ERCP) fails or is contraindicated. Despite emerging evidence favoring primary stenting, data directly comparing it with the traditional two-staged drainage approach remain scarce.PurposeTo evaluat...

“Endoscopic ultrasound-guided choledochoduodenostomy versus endoscopic retrograde cholangiopancreatography for primary biliary decompression in distal malignant biliary obstruction: a randomized comparative study”

EUS-BD showed similar results regarding technical and clinical success, procedure time, overall adverse events, and reinterventions rates as a method of primary biliary decompression in cases of unresectable distal malignant biliary obstruction compared to ERCP-BD.
