De novo AIH presenting concurrently with advanced HIV and profound immunosuppression is exceptionally rare, with few reported cases in the literature, and diligent investigation can uncover rare but treatable causes of liver dysfunction.
Abstract
We report a woman in her 50s who presented with jaundice and severe hepatocellular injury, concurrently found to have newly diagnosed advanced HIV infection (CD4 count 17, viral load 195 600 copies/mL). In this context, infectious hepatitis or opportunistic infections were the important initial consideration, but viral serology, imaging and liver biopsy excluded opportunistic and viral causes. Markedly raised immunoglobulin G, positive anti-nuclear and smooth muscle antibodies, typical histology and rapid response to corticosteroids confirmed acute autoimmune hepatitis (AIH). She improved on prednisolone, transitioned to azathioprine and commenced antiretroviral therapy with subsequent normalisation of liver function and undetectable HIV viral load. De novo AIH presenting concurrently with advanced HIV and profound immunosuppression is exceptionally rare, with few reported cases in the literature. It highlights that, even in this paradoxical setting, diligent investigation can uncover rare but treatable causes of liver dysfunction.
Background In people living with HIV, syphilis may present atypically and can involve the liver, making the etiology of liver injury difficult to determine. Primary biliary cholangitis (PBC), an autoimmune cholestatic liver disease, has rarely been reported in patients with HIV infection. Case Presentation A 34-year-ol...

Background Disseminated herpes simplex virus (HSV) infection is an uncommon but serious condition, typically seen in immunocompromised hosts. Cytopenias, including neutropenia and thrombocytopenia, are rare manifestations, particularly in patients not actively receiving immunosuppressive therapy. Case Presentation We r...

The coexistence of Systemic Lupus Erythematosus (SLE) and Human Immunodeficiency Virus (HIV) infection represents a rare and complex clinical entity due to opposing immunological mechanisms. While HIV leads to immunodeficiency through CD4+ T-cell depletion, SLE is characterized by immune hyperactivity and autoantib...

BACKGROUND
Hepatitis E virus (HEV) remains endemic worldwide, especially in resource-constrained settings. Although HEV-associated Guillain-Barré syndrome (GBS) is well documented in adults, severe presentations in adolescents are scarcely reported.
