Abstract
Highlights • Early hscTrop I levels evaluated for NSTEMI diagnosis in non-CAD early presenters.• In this pilot study, a 1-hour absolute delta hs-cTnI of 4 ng/L showed 95% specificity for NSTEMI, though this finding requres validation.• Baseline hscTrop I < 3 ng/L achieved 86% NPV for ruling out early NSTEMI.• Delta hscTrop I was the strongest predictor with an AUC of 0.796.• Standard cut-offs may miss early NSTEMI in patients without prior heart disease.
