Frailty and pre-frailty substantially affect pain, mobility, function, and quality of life in older adults with KOA and future high-quality longitudinal and intervention research is essential to elucidate causal mechanisms and establish evidence-based frailty management strategies tailored to KOA populations.

Abstract

Background: Frailty, an age-related syndrome with reduced physiological reserve, compounds KOA-related pain, disability, and functional decline. Evidence heterogeneity limits clear understanding of relationships and optimal interventions. Objective: To synthesize evidence on frailty among older adults with knee osteoarthritis (KOA) regarding prevalence, assessment, risk factors, and functional outcomes. Methods: Scoping review following PRISMA-ScR principles. Searches of Scopus, Web of Science, and PubMed (2010–2025) showed 101 records; 10 studies met inclusion criteria (cross-sectional, longitudinal, and one randomized controlled trial). Results: Frailty prevalence ranged from 4.39% to 12.9%, while pre-frailty prevalence reached 65.6% among community-dwelling older adults with KOA. Consistent risk factors included older age, female sex, severe knee symptoms, obesity, malnutrition, comorbidities, functional impairment, depression, low physical activity, and socioeconomic disadvantage. Longitudinal evidence showed bidirectional relationships: frailty predicted worse pain trajectories, and KOA increased incident frailty. Moderate-to-vigorous physical activity was associated with better symptoms; sedentary behavior was linked to worse stiffness. Chair yoga reduced pain, particularly in frailer participants, though short-term studies did not prove significant frailty improvement. Conclusions: Frailty and pre-frailty substantially affect pain, mobility, function, and quality of life in older adults with KOA. Clinical recommendations include: 1) routine frailty screening; 2) early pre-frailty identification; 3) multidomain interventions encompassing physical activity, muscle strengthening, nutrition optimization, pain management, and psychosocial support. Future high-quality longitudinal and intervention research is essential to elucidate causal mechanisms and establish evidence-based frailty management strategies tailored to KOA populations.

The Predictive Value of The 9-Point Clinical Frailty Scale on Outcomes in Older Adults with Hip Fracture: A Systematic Review

Abstract Hip fracture is a major cause of morbidity, mortality, and functional decline among older adults. Frailty has emerged as an important prognostic factor in this population, and the Clinical Frailty Scale (CFS) offers a rapid and practical method for assessing frailty. This systematic review assessed the prognos...

Frailty-targeted cardiovascular rehabilitation and management in older adults with cardiovascular disease and frailty: a systematic review and meta-analysis.

The evidence suggests favourable but uncertain signals for selected proximal functional outcomes, with no clear benefit for distal clinical outcomes, given the limited evidence base, risk of bias, heterogeneity, imprecision, and very low certainty of evidence.

Long COVID in Older Adults: A Geriatric Framework for Pathophysiology, Frailty, Functional Decline, and Clinical Management—A Narrative Review

A hypothesis-generating, conceptual and pragmatic geriatric framework that integrates biological aging, frailty, nutrition, cognition, functional status, and long-term care into assessment and management is proposed and is intended to complement, not replace, WHO/CDC symptom-based definitions.

The effectiveness of exercise on physical and cognitive function in older adults with pre-frailty or frailty: A systematic review and meta-analysis.

BACKGROUND Frailty is a geriatric syndrome characterized by concurrent physical and cognitive decline. We evaluated exercise effectiveness on these domains in pre-frail and frail older adults. METHODS We searched four databases to 8 June 2026 for randomized controlled trials (RCTs). Meta-analyses utilized the DerSimo...

Frailty across Hip arthroplasty pathways: vulnerability domains, outcomes, and implications for care—a narrative review

A clinically useful pathway should combine low-burden screening with targeted vulnerability-domain assessment, incorporate procedure context, and repeat assessment during recovery, to guide proportional monitoring, rehabilitation, discharge planning, and shared decisions rather than serve as an isolated threshold for d...
