Early EASIX levels are independently associated with 7-day and 30-day mortality in delirium patients and may serve as a potential adjunctive marker for risk assessment in this population of adults diagnosed with delirium.

Abstract

Objective In intensive care units (ICUs), delirium frequently occurs as an acute brain dysfunction and is linked to a higher risk of death. The Endothelial Activation and Stress Index (EASIX)—derived from three routine lab tests: lactate dehydrogenase, serum creatinine, and platelet count—has been proposed as a marker of endothelial injury and multi-organ failure. We sought to determine whether the EASIX score is associated with short-term mortality among patients with delirium. Methods We conducted a retrospective cohort study using data from the Medical Information Mart for Intensive Care IV database. The analysis included adults diagnosed with delirium. EASIX values were computed from measurements taken within the first 24 hours of ICU admission and then log2-transformed. The primary endpoint was all-cause mortality at 7 days; the secondary endpoint was all-cause mortality at 30 days. To evaluate the association, we used Cox proportional hazards models, restricted cubic splines, subgroup analyses, and inverse probability of treatment weighting (IPTW). Results Among 8,378 delirium patients, 966 (11.53%) died within 7 days, and 1,893 (22.59%) died within 30 days. Higher log2-EASIX was independently associated with increased 7-day mortality (fully adjusted HR = 1.940, 95% CI: 1.574–2.391, p < 0.001) and 30-day mortality (HR = 1.720, 95% CI: 1.485–1.994, p < 0.001). A linear dose-response relationship was observed. Subgroup analyses for the 7-day mortality endpoint showed that the association was stronger in patients aged <60 years and those without sepsis (interaction P < 0.05 for both). The results remained robust after IPTW. Conclusions Early EASIX levels are independently associated with 7-day and 30-day mortality in delirium patients. As a simple score derived from routine laboratory parameters, the EASIX may serve as a potential adjunctive marker for risk assessment in this population.

Association between endothelial activation and stress index and all-cause mortality risk in cerebral infarction patients

Acute cerebral infarction (ACI) is a major cause of global morbidity and mortality, with endothelial dysfunction being a key factor in its pathophysiology. The Endothelial Activation and Stress Index (EASIX) is a novel biomarker reflecting endothelial injury and systemic stress; however, its prognostic role in mortalit...

Modified Endothelial Activation and Stress Index (mEASIX) Predicts Long-Term Mortality in Patients with Acute Coronary Syndromes

Endothelial dysfunction, inflammation, and microvascular injury are key mechanisms involved in the development and progression of acute coronary syndrome (ACS). The Endothelial Activation and Stress Index (EASIX) and its derivatives have emerged as readily available biomarkers of endothelial stress; however, the progno...

Elevated blood urea nitrogen-to-albumin ratio is associated with incident sepsis-associated delirium: evidence from a weighted competing-risk analysis in a multicenter cohort study

Background Sepsis-associated delirium (SAD) is a frequent manifestation of acute brain dysfunction in sepsis. Although the blood urea nitrogen-to-albumin ratio (BAR) has been associated with prognosis in critically ill patients, its relationship with incident SAD remains uncertain. We evaluated the association between...

Admission Endothelial Activation and Stress Index in Acute Myocardial Infarction: Incremental Value Beyond the GRACE Score and a Test of the Index Weights

Background/Objectives: The Endothelial Activation and Stress Index (EASIX), calculated from lactate dehydrogenase, creatinine and platelet count, has been linked to mortality in cardiovascular cohorts. We examined whether admission EASIX adds prognostic information to the Global Registry of Acute Coronary Events (GRACE...

Prognostic Value of the Modified Endothelial Activation and Stress Index (m-EASIX) for 90-Day Mortality in Patients with Moderate-to-Severe Hypercalcemia of Malignancy

Background: Hypercalcemia of malignancy (HCM) is associated with poor short-term prognosis, particularly when it represents the initial manifestation of an underlying, as yet uncharacterized malignancy. Serum calcium concentration alone may not adequately capture the systemic biological burden associated with advanced...
