Abstract
Background
Early language delay is a common presenting concern among toddlers with autism spectrum disorder (ASD) and developmental language disorder (DLD), creating challenges for clinical assessment and referral decisions. Although ASD and DLD differ in core social-communicative features, restricted/repetitive behaviours, and developmental mechanisms, language-focused assessment may provide complementary information about early communicative profiles. However, culturally adapted tools for assessing early language and communication in Mandarin-speaking toddlers remain limited.
Methods
A total of 213 children aged 24-36 months (ASD = 89, DLD = 60, TD = 64) were recruited from the Child Healthcare Department of a tertiary hospital between 2020 and 2022. Clinical diagnoses were established by multidisciplinary teams based on DSM-5 and CATALISE criteria. Caregivers completed the Diagnostic Receptive and Expressive Assessment of Mandarin-Infant & Toddler (DREAM-IT), which evaluates receptive and expressive language, cognitive play, and social communication. Receiver operating characteristic (ROC) curve analyses assessed discriminative validity across diagnostic groups, logistic regression examined the contribution of DREAM-IT domains to ASD-versus-DLD classification, and concurrent validity was evaluated against GDS, ABC, CARS, and ADOS-2.
Results
DREAM-IT showed strong diagnostic accuracy in distinguishing children with ASD or DLD from TD children (AUC = 0.84-0.998), but its ASD-versus-DLD discrimination was domain-specific and more limited. Receptive language provided the strongest ASD-versus-DLD discrimination (AUC = 0.81; sensitivity = 69.1%; specificity = 84.3%; optimal cutoff = 86.5), whereas expressive language, cognitive play, and social communication showed poor-to-limited standalone performance (AUC = 0.51-0.57). Logistic regression indicated that receptive language (OR = 1.10, p < 0.05) and expressive language (OR = 0.77, p = 0.015) independent contributed to ASD-versus-DLD discrimination, with an overall classification accuracy of 79.2%. DREAM-IT classifications demonstrated moderate-to-substantial agreement with established developmental and autism-related instruments, including GDS, ABC, CARS, and ADOS-2 (Cohen's κ = 0.62-0.82). Developmental age comparisons revealed domain-specific lags, with ASD characterized by greater receptive and social delays and DLD showing the largest lag in expressive language.
Conclusions
The Mandarin DREAM-IT provides clinically useful information about early language and communication development in Mandarin-speaking toddlers with ASD and DLD and shows moderate-to-strong concurrent alignment with established developmental and autism-related measures. Its primary value lies in characterizing domain-specific developmental patterns and offering useful complementary information within a broader, multi-method diagnostic framework for Mandarin-speaking populations.

What is already known on this subject Differentiating autism spectrum disorder (ASD) from developmental language disorder (DLD) in toddlers is clinically important but challenging. Both conditions may present with early language delay, while ASD is characterised by core social-communication difficulties and restricted or repetitive behaviours. Accurate interpretation therefore requires a multi-method assessment integrating language evaluation, developmental history, direct observation, and autism-specific measures. However, culturally and linguistically appropriate tools for characterising early language and communication in Mandarin-speaking toddlers remain limited. What this study adds to the existing knowledge This study evaluated the diagnostic accuracy and concurrent validity of the Mandarin DREAM-IT in 213 toddlers with ASD, DLD, or typical development. DREAMIT showed good-to-excellent accuracy in distinguishing children with ASD or DLD from typically developing children, but ASD-versus-DLD discrimination was more limited and domain-specific. Receptive language provided the clearest discriminatory signal, whereas expressive language, cognitive play, and social communication showed substantial overlap. DREAM-IT developmental age scores showed moderate correlations with corresponding GDS measures, and its descriptive rule-based patterns showed substantial agreement with autism-specific instruments. What are the potential or actual clinical implications of this study? DREAM-IT may provide useful caregiver-reported information about receptive language, expressive language, cognitive play, and social communication in Mandarin-speaking toddlers. Its main clinical value lies in characterising domain-specific developmental patterns and providing complementary information within a broader, multi-method assessment framework. It should not be used as a standalone instrument to diagnose ASD or DLD or to differentiate between them. Results should be interpreted alongside developmental history, direct clinical observation, developmental testing, and autism-specific measures.

Autism Spectrum Disorder (ASD) and Developmental Language Disorder (DLD) are both associated with language difficulties, yet the extent to which they share common structural language profiles remains unclear. Previous research has reported both overlap and divergence across phonological, morphosyntactic, semantic, and narrative domains, creating challenges for differential diagnosis and intervention planning.
This systematic review aimed to synthesize empirical evidence comparing structural language abilities in children with ASD and DLD and to identify areas of convergence and divergence across language domains.
A systematic literature search was conducted in PubMed, ERIC, PsycINFO, Google Scholar, and Semantic Scholar following PRISMA guidelines. Studies published in English between 2013 and 2025 were eligible if they included children aged approximately 3–18 years with ASD and DLD and examined structural language domains, including phonology, morphology, syntax, semantics, or narrative measures. 44 studies met the inclusion criteria and were synthesized using a narrative approach due to methodological heterogeneity.
The included studies suggested overlapping performance between children with ASD accompanied by language impairment (ASD-LI) and children with DLD across receptive and expressive language, morphosyntax, sentence repetition, nonword repetition, and complex syntactic processing. Overlapping performance was particularly evident in grammatical morphology, tense and aspect marking, clitic production, and comprehension of syntactically demanding structures. However, important differences emerged. Children with DLD generally exhibited more consistent and pervasive structural language deficits, whereas language performance in ASD was characterized by greater heterogeneity. Difficulties in ASD were more frequently influenced by pragmatic, attentional, cognitive, and processing-related factors, and atypical error patterns were observed in several linguistic domains.
The available evidence suggests a partially overlapping structural language phenotype between children with ASD and co-occurring language impairment (ASD-LI) and those with DLD, while also highlighting important disorder-specific characteristics. Across several structural language domains, the included studies frequently reported similar performance on standardized language measures. The findings further suggest that similar behavioral performance may reflect distinct underlying linguistic and cognitive mechanisms, resulting in different patterns of language impairment. Overall, the evidence suggests that ASD-LI and DLD share several structural language characteristics but do not represent a single, unified language disorder. These findings have important implications for differential diagnosis, language assessment, and the development of targeted intervention approaches for children with ASD and DLD.

Introduction: Autism spectrum disorder (ASD) is diagnosed with impairment in social interactions and communication along with the presence of restricted repetitive behaviour (RRB). These stereotypies can have a significant impact on the quality of life (QoL) of both the affected children and caregivers.Objectives: To characterize RRBs in children with ASD according to the four DSM-5 Section B domains. Specific objectives were to quantify the frequency and severity of RRB, to examine its associations with clinician-rated ASD severity and attention difficulties and to assess its relationships with the functioning of the child health-related QoL, social communication, and parental perceptions regarding the child’s condition and the impact on daily life.Method: A descriptive cross-sectional study was conducted over 6 months at Teaching Hospital Kurunegala. Children diagnosed with ASD and aged 2-17 years were included. Statistical analysis was done using SPSS-25; tests for associations were done via t-test and Pearson’s correlation; significance was set at p-value of 0.05.Results: The study included 94 children with a mean age of 6.62 years and 74.5% were males. The most frequently observed RRB was pacing or moving around repetitively. Mean PedsQL score was 64.55. Moss attention rating scale (MARS) had a mean score of 64.00; 81.1% believed that doctors could help improve their child’s condition. No significant correlation was found between RRB and any component of PedsQL. Physical functioning was significantly associated with social functioning, emotional functioning, and school functioning. Social communication severity score was significantly associated with physical functioning, emotional functioning, and social functioning.Conclusions: The most frequently observed RRB was pacing or moving around repetitively. No significant correlation was found between RRB and any component of PedsQL. Physical functioning was significantly associated with social functioning, emotional functioning, and school functioning. Narrative skills are important for communication and everyday interaction. This study examined how Mandarin-speaking children with and without autism spectrum disorder (ASD) produce oral narratives. = 0;4) who were matched on chronological age and expressive lexical ability. Children completed picture-based storytelling tasks from the Multilingual Assessment Instrument for Narratives. Narrative production was analyzed in three dimensions: macrostructure (global organization of the story), microstructure (internal linguistic units), and internal state terms (ISTs). Results showed that children with ASD did not differ significantly from their TD peers in Attempt, IST diversity, or basic productivity measures, including the total number of utterances, the number of different words, and the overall frequency of conjunctions. In contrast, the ASD group showed significantly lower performance on story structure, story complexity, Goal, and Outcome, and produced fewer total words, shorter mean length of utterance in words, and fewer complex sentences. These findings indicate that Mandarin-speaking children with ASD show narrative difficulties in production, with particular weaknesses in macrostructural organization and specific aspects of microstructure. The results add to cross-linguistic evidence on autism-related narrative challenges and highlight the need for interventions in Mandarin that target episode organization, narrative elaboration, and syntactic complexity.
