The results suggest that metabolic stress can aggravate dysregulated myocyte Ca2+ handling and promote cardiac dysfunction in CPVT hearts.

Abstract

Abstract Catecholaminergic polymorphic ventricular tachycardia (CPVT) is a genetic arrhythmic syndrome caused by mutations in the sarcoplasmic reticulum (SR) calcium (Ca2+) release channel RyR2 or its accessory proteins, including calsequestrin‐2 (CASQ2). RyR2 dysfunction or leak is the hallmark feature of CPVT and directly triggers arrhythmias. Despite this electrical instability, CPVT hearts are typically anatomically normal and lack pathological remodelling. Therefore, we hypothesized that introducing additional stressors, such as metabolic stress, would worsen cardiac arrhythmias and potentially trigger pathological remodelling. To test this hypothesis, metabolic stress was introduced by crossbreeding a CPVT model of CASQ2−/− mice with the Akita mouse, a genetic model of diabetes that also lacks cardiac remodelling. We found that dysregulation of myocyte Ca2+ handling was exacerbated in Akita‐CASQ2−/− cells, which exhibited slower Ca2+ transient decay kinetics and increased arrhythmogenic Ca2+ waves. Akita‐CASQ2−/− mice also displayed more severe arrhythmias, diastolic dysfunction, and cardiac hypertrophy, but without evidence of fibrosis, cell death, or worsened metabolic dysfunction. Finally, mitochondrial reactive oxygen species production was increased in Akita‐CASQ2−/− cardiomyocytes, which may contribute to the more severe pathological phenotype. Overall, our results suggest that metabolic stress can aggravate dysregulated myocyte Ca2+ handling and promote cardiac dysfunction in CPVT hearts.

Connexin-43 remodeling drives atrial arrhythmias in hypothyroidism: ranolazine as a potential therapeutic strategy.

Hypothyroidism is associated with cardiovascular complications, including supraventricular arrhythmias. Thus, this study investigated functional and morphological alterations in the atrial of hypothyroid rats and evaluated the effects of acute ranolazine treatment on arrhythmia susceptibility. Hypothyroidism was induce...

More direct approaches, including modulation of Ca2+-handling proteins, ryanodine receptor 2 (RyR2) stabilization, sarcoplasmic reticulum Ca2-ATPase 2a (SERCA2a) restoration, and repair of T-tubule-associated microdomains, remain largely preclinical or early translational.

Congestion as an electrophysiological state in heart failure: from mechano-electric coupling to dynamic arrhythmic risk.

Heart failure (HF) is traditionally characterized by hemodynamic impairment and neurohormonal activation, with congestion representing a central clinical feature. However, the electrophysiological consequences of congestion remain underexplored. Emerging evidence suggests that volume overload and elevated filling press...

PFKFB2 Gates a Relationship Between Cardiac Glycolytic Regulation and Electrophysiological Function.

BACKGROUND The cardiac isoform of PFKFB2 (phosphofructokinase-2/fructose 2,6-bisphosphatase) is the heart's strongest glycolytic regulator but is degraded in the absence of insulin signaling. This makes PFKFB2 loss critical to understand in metabolic heart disease, of which impaired insulin signaling is a hallmark. Pro...

Mechanisms and Advances in the Treatment of Arrhythmias Associated With Pulmonary Hypertension

Arrhythmias related to pulmonary hypertension (PH) are major complications of disease progression and are associated with a significantly increased risk of death. The pathophysiology of these arrhythmias involves complex, multifaceted mechanisms. Arrhythmic substrates include electrical remodeling (ion channel imbalanc...
