Subgroup analyses demonstrated significantly greater improvement in paced QRS duration and LVEF in the patient subgroups under 70 years of age and those with cardiac resynchronization therapy (CRT) indications.

Abstract

Background Left bundle branch pacing (LBBP) is proposed as a superior physiological pacing method to left ventricular septal pacing (LVSP). However, its comparative benefits on hard clinical outcomes remain inadequately defined. Methods We systematically searched four databases for studies comparing LBBP with LVSP. The primary outcomes were all-cause death or heart failure hospitalization (HFH), all-cause death, HFH, or malignant ventricular arrhythmias. Subgroup analyses were performed to identify effect modifiers for the paced QRS duration and left ventricular ejection fraction (LVEF) at follow-up. Results Fourteen studies with 1,592 patients were included. LBBP was associated with reduced risks of all-cause death/HFH (risk ratio [RR] = 0.25), all-cause death (RR = 0.30), HFH (RR = 0.26), and malignant ventricular arrhythmias (RR = 0.36). LBBP also resulted in a QRS narrowing and greater LVEF improvement. Subgroup analyses demonstrated significantly greater improvement in paced QRS duration and LVEF in the patient subgroups under 70 years of age and those with cardiac resynchronization therapy (CRT) indications. Conclusions LBBP may be superior to LVSP, reducing mortality, heart failure, and arrhythmic events. Younger patients and those requiring CRT derive the greater benefit. These promising findings support LBBP as a preferred physiological pacing strategy, but large-scale randomized trials are needed to confirm these results. Comparative clinical outcomes of stylet-driven left bundle branch area pacing versus right ventricular pacing in patients with preserved ejection fraction: A 3-year follow-up study

Chronic right ventricular pacing (RVP) can exacerbate heart failure, but the clinical impact of left bundle branch area pacing (LBBAP) in patients with preserved left ventricular ejection fraction (LVEF) remains unclear. Because LBBAP has been performed almost exclusively with stylet-driven leads (SDLs) in Korea, we co...

Left Bundle Branch Area Pacing: Current Clinical Practice and Future Perspectives.

Conventional right ventricular pacing (RVP) is associated with electromechanical dyssynchrony and may contribute to pacing-induced cardiomyopathy (PICM). Left bundle branch area pacing (LBBAP) has emerged as a physiological pacing strategy that engages the native conduction system. This review summarizes the current cl...

Incidence of Atrial Fibrillation in Pacemaker Patients with Left Bundle Branch Area Pacing Versus Right Ventricular Pacing Leads.

BACKGROUND Guidelines and early clinical data indicate left bundle branch area pacing (LBBAP) may help avoid pacing induced cardiomyopathy, but the impact of LBBAP on atrial fibrillation (AF) is less certain. OBJECTIVE To compare new onset AF in patients with LBBAP leads with RVP leads. METHODS RVP leads (N=5225) a...

Left Bundle Branch Area Pacing Versus Biventricular Pacing for Cardiac Resynchronization Therapy: A Meta-Analysis of RCTs.

BACKGROUND Left bundle branch area pacing (LBBAP) has emerged as an alternative to conventional biventricular pacing (BiVP) for cardiac resynchronization therapy (CRT). While observational studies have suggested potential advantages of LBBAP in some domains. However, recent randomized controlled trials (RCTs) have repo...

Differences in Tricuspid Valve Insufficiency, Mechanical Synchrony, and Cardiac Remodeling Between Left Bundle Branch Area Pacing and Right Ventricular Pacing.

BACKGROUND Cardiac implantable electronic device-induced tricuspid regurgitation (CIED-induced TR) involves primary (lead-induced) and functional etiologies. The impact of left bundle branch area pacing (LBBAP) versus right ventricular pacing (RVP) on these mechanisms remains unclear. OBJECTIVE To compare the inciden...
