Abstract

Background: Cognitive impairment in Parkinson disease (PD) significantly affects patients ability to perform instrumental activities of daily living (IADLs). However, neuropsychiatric symptoms, which are highly prevalent in PD, may also contribute to functional impairments. In this study, we investigated the independent contributions of cognitive and neuropsychiatric symptoms to functional impairments and tested whether mood mediates cognition-function associations. Methods: We examined 104 patients from the Quebec Parkinson Network and replicated analyses in a sample of 144 patients from an independent online cohort. Participants completed the Penn Parkinsons Daily Activities Questionnaire (PDAQ), a validated patient-reported measure designed to capture cognition-dependent IADLs, standard cognitive tests (executive function, attention, working memory), and neuropsychiatric questionnaires (apathy, depression, impulsivity, and anhedonia). Regression models tested the independent and combined effects of cognition and neuropsychiatric symptoms on PDAQ; mediation analyses explored mechanistic pathways. Results: Both cognitive performance and neuropsychiatric symptoms were independently associated with PDAQ scores and the proportion of variance explained by neuropsychiatric symptoms was comparable to that explained by cognitive performance. Furthermore, depression and impulsivity partially confounded the relationship between cognitive performance and PDAQ and impulsivity partially mediated this relationship. The independent associations between neuropsychiatric symptoms and PDAQ were replicated in the replication sample. Conclusion: These results indicate that impairments in cognitive IADLs reflect contributions of both cognition and neuropsychiatric changes and suggest that trials using measures of IADLs as an outcome measure should also carefully assess and control for neuropsychiatric symptoms.

Disease-specific roles of multidimensional apathy in linking cognitive and social functioning across neurodegenerative diseases.

BACKGROUND Apathy is a multidimensional neuropsychiatric syndrome involving cognitive, emotional, and behavioral domains. Cognitive impairment and apathy are both associated with reduced social functioning, but how specific apathy dimensions relate cognition to social functioning across neurodegenerative diseases remai...

A-04: Functional Impairment in MCI: Memory and Apathy Predict Presence, Executive Function Relates to Severity

Objective: Findings regarding the contributions of specific cognitive domains and neuropsychiatric symptoms to functional impairment (FI) in mild cognitive impairment (MCI) are mixed. Additionally, measures of FI are often highly skewed in MCI samples, with many individuals reporting no impairment, complicating stand...

A-09: Intra-Individual Cognitive Variability as a Mediator of Associations Between MCI and Cognitive Performance in Older Adults With and Without Multiple Sclerosis

Objective: Intra-individual cognitive variability (IICV) is an important marker of cognitive functioning in aging and neurological disease. Whether IICV may serve as a pathway to explain the effect of mild cognitive impairment (MCI) on cognition has not been reported. Therefore, we investigated whether IICV mediated...

Associations between cognition and functioning: examining the role of mental health in clinically ascertained and population-based samples

Background Cognitive function is a significant predictor of daily functioning in the population, yet the extent to which common mental health symptoms account for this relationship remains unclear. Aims This study aimed to examine the relationship between cognition, mental health and functioning across two cohorts. Met...

Subjective Cognitive Concerns and Cognitive Trajectories in Parkinson's Disease: Biomarker Impact

SCC is related to AD biomarkers and accelerated cognitive decline in the earliest stages of PD, and may serve as an early marker of cognitive risk, especially among persons with greater AD‐ or PD‐related burden.
