Osteoporosis-related knowledge in a German adult population: determinants and translational implications for fragility fracture prevention.

Abstract

Purpose

Osteoporosis is highly prevalent and a major cause of fragility fractures, particularly of the spine. Adequate patient knowledge is essential for prevention, early diagnosis, and treatment adherence. This study aimed to assess osteoporosis-related knowledge in an adult population and to identify demographic and clinical factors associated with knowledge levels.

Methods

A cross-sectional online survey was conducted within a German patient panel. Adults (≥ 18 years) who provided electronic consent were included. Osteoporosis knowledge was assessed using the validated Osteoporosis Knowledge Assessment Tool (OKAT) and Facts on Osteoporosis Quiz (FOOQ). Osteoporosis risk was descriptively evaluated using the IOF One-Minute Osteoporosis Risk Test. Participants were stratified into three age groups (< 50, 50-65, > 65 years). Group comparisons were performed using Mann-Whitney U tests; associations were analyzed using Spearman correlations. Additionally, multivariable linear regression analyses were performed.

Results

A total of 315 participants were included (mean age 61.6 ± 12.0 years; 74.3% female). IOF risk scores increased significantly with age. Bivariate analyses demonstrated significantly higher knowledge in participants < 50 years compared to older groups, but this age association was no longer observed after multivariable adjustment. Women scored higher than men on the FOOQ but not on the OKAT. Higher educational attainment and employment in the medical field were significantly associated with higher knowledge scores. Neither personal diagnosis nor family history of osteoporosis was associated with increased knowledge. IOF risk scores showed a weak negative correlation with FOOQ scores.

Conclusion

Educational level emerged as an important correlate of osteoporosis knowledge in this exploratory analysis, whereas personal or familial disease experience did not improve understanding. These findings highlight a relevant gap in osteoporosis education and support the need for future targeted educational interventions, including in patient populations at risk for vertebral fragility fractures.

Osteoporosis in internal medicine outpatients: Sex-specific disparities in prevalence, risk stratification, and treatment in a large cohort

Osteoporosis is a major health problem linked to population aging. Internal medicine outpatients often have multiple comorbidities and a high osteoporosis risk; however, osteoporosis prevalence and treatment rates remain unclear. We investigated osteoporosis prevalence and treatment rates in a large cohort of internal medicine outpatients. This single-center, cross-sectional study used baseline data from the ongoing JUSTICE-TOKYO prospective cohort study ( N  = 1,078) of Japanese internal medicine outpatients aged ≥ 65 years. Demographic and clinical characteristics, osteoporosis prevalence, treatment rates, and risk assessment (Osteoporosis Self-assessment Tool for Asians [OSTA]) were assessed. Student’s t-test was used for between-sex comparisons of continuous variables and Pearson’s chi-square test for comparisons of categorical variables. Multivariable logistic regression analysis was performed to identify factors independently associated with osteoporosis. After excluding missing data, 1,040 cases were analyzed. The mean age was 78.2 ± 6.1 years. Overall osteoporosis prevalence was 28.3%, and differed significantly between men (15.8%) and women (38.0%) ( p  < 0.001). The treatment rate was 29.3% among osteoporosis patients, with a significant difference between men (12.5%) and women (34.7%) ( p  < 0.001). In the primary multivariable model adjusted for age, sex, and body mass index (BMI), older age, female sex, and lower BMI were independently associated with osteoporosis. In an exploratory model additionally including grip strength, lower grip strength was independently associated with osteoporosis, whereas the association with female sex was attenuated after adjustment. In this cohort, osteoporosis was highly prevalent and substantially undertreated, particularly among men. Significant sex-specific disparities were observed in both osteoporosis prevalence and treatment rates.

Screening based on FRAX and a history of previous fractures is an effective strategy for reducing fracture rates in real-world clinical practice and may therefore be recommended for widespread use in primary healthcare.

Osteoporosis Knowledge of Older Women with Fragility Fractures: A Cross-Sectional Study Based on Latent Profile Analysis

Objective This study aimed to investigate osteoporosis knowledge (OK) profiles among older women with fragility fractures using latent profile analysis, identify subgroup characteristics, and explore associated factors. Methods A cross-sectional survey was conducted among 9212 older women with fragility fractures covering 31 provinces in China from September to November 2023. The survey utilized a self-designed general information questionnaire and the Chinese version of the Osteoporosis Knowledge Assessment Tool. Univariate analysis and logistic regression were employed to analyze associated factors. Latent Profile Analysis was applied to classify the older women with fragility fractures based on their OK levels, and multinomial logistic regression was used to identify factors influencing their OK levels. Results Older women with fragility fractures were classified into five distinct latent profiles according to their OK levels: Low OK group (9.9%), Medium-Low OK group (11.1%), High OK group (50.9%), Medium OK group (18.8%), and High awareness and low management group (9.3%). Key factors influencing the classification of OK levels included marital status, BMI, education level, filial care, postoperative self-care ability, receipt of anti-osteoporosis therapy, use of calcium or vitamin D supplements, perceived necessity of fall prevention, receipt of health education, content of health education, and methods of health education delivery. Conclusion The OK levels of older women with fragility fractures can be stratified into five distinct profiles. Understanding the heterogeneity of OK levels and its associated factors among this population offers valuable insights for healthcare professionals to formulate targeted interventions. This study makes a significant contribution to the advancement of secondary prevention strategies for osteoporosis.

Sarcopenia risk in women with postmenopausal osteoporosis: a SARC-F-based assessment

Background/aim Osteoporosis and sarcopenia are diseases of bone and muscle tissue, respectively. Studies reported that these disorders were interrelated. However, this relation was explored in older adults and/or with certain comorbidities, and information about nonelderly postmenopausal women is scarce. This study’s objectives were to investigate the relationship between osteoporosis and SARC-F-defined sarcopenia risk in postmenopausal women aged ≤70 years and to examine the effect of other factors on this risk. Materials and methods This multicenter case-control study included postmenopausal women aged 45–70 years. Dual-energy X-ray absorptiometry was utilized to diagnose osteoporosis. Sociodemographic and clinical variables, sarcopenia risk (SARC-F), muscle strength (grip strength and five-times sit-to-stand test: 5T-STS), and physical activity levels (International Physical Activity Questionnaire-Short Form) were compared between osteoporotic and nonosteoporotic participants. Results A total of 119 osteoporotic (median age: 58 years) and 119 nonosteoporotic (median age: 59 years) participants were recruited. No differences were observed between the osteoporotic and nonosteoporotic groups regarding SARC-F-defined sarcopenia risk (48.7% vs. 38.7%, p = 0.117), probable sarcopenia (14.5% vs. 15.3%, p = 0.876), grip strength (23 vs. 22 kg, p = 0.329), 5T-STS (13.90 vs. 13.96 s, p = 0.986), or physical activity levels (907.5 vs. 792 MET-min/week, p = 0.983). Although educational level [odds ratio (OR): 0.366, 95% confidence interval (CI): 0.189–0.707, p = 0.003], proton pump inhibitor (PPI) use (OR: 2.494, 95% CI: 1.227–5.068, p = 0.012), and physical activity level (OR: 0.966, 95% CI: 0.945–0.988, p = 0.003) were independent predictors of SARC-F-defined sarcopenia risk, osteoporosis was not (p = 0.176). Conclusion SARC-F-defined sarcopenia risk was not increased in women with postmenopausal osteoporosis aged ≤70 years compared to nonosteoporotic controls. Lower educational status, PPI use, and low physical activity levels were associated with an increased SARC-F-defined sarcopenia risk.
